Well, PSMA Pylarify scan yesterday was basically unremarkable EXCEPT for the following: Lymph Nodes: New radiotracer avid subcentimeter right common iliac node (image266) and a subcentimeter right pelvic sidewall/obturator lymph node (image 295).

Finally getting a PSMA Pylarify test after a PSA rise from 0.64 to 0.90 in 11 weeks. Should be interesting. No symptoms from the cancer so far. At the current PSA level it would appear there is an almost 90% chance of locating cancer somewhere.

A Pylarify PSMA PET/CT showed a "small metabolically active left presacral LN .7 x .9 cm max SUV 3.1 My RO is recommending 6 months of Orgovyx concurrent with pelvic RT beginning at the same time and asap.

I just had my 3rd PSMA-PET, and was relieved but surprised that it was totally clear. I had expected mets similar to my 1st one which was also run at a PSA of 4. BTW, the Axumin scan was clear at that time. I've been on an ADT/Zytiga vacation for about 18 months. My PSA did become undetectable for

Pylarify scan revealed apparent mets on spine, ribcage, pelvis in small numbers. His urologist suggests lutron+ docetaxel. This is a relatively rural area far from big cancer centers. Is this treatment optimal? Can he/ should also get radiation treatments for the detected lesions?

July 2022 w/ PSA @ 0.95, Pylarify PSMA scan shows intense focal uptake (SUV 20.2) in L apex of prostate only. 3T Dotarem MRI shows corresponding 0.9 cm lesion suspicious for tumor and diffusely decreased T2 signal intensity in prostate likely due to post radiation TX changes.

The new Oncologist is suggesting a Pylarify PET scan for PSMA. My last Axumin scan in 2020 showed PC in multiple lower abdominal and bilateral pelvic lymph nodes and prostate. Is this scan a good idea for me at this point?

Most recent was pylarify PET scan. Impression: No evidence of psma avid malignancy. MO is recommending a treatment break at this point. Best case scenario testosterone coming back slowly and psa staying undetectable for a lengthy duration.

Pylarify PET scan shows numerous stable non tracer avid sclerotic osseous lesions on the CT portion of the exam favoring treated metastases. Nice, but new small focus within a lesion at the left acetabulum consistent with active metastasis. That lesion is scheduled for radiation next week.

If insurance covers both, at the same psa number which scan is more sensitive to locate bone metastases or soft tissue/ lymph node metastases. If there have been any comparison studies/ data please share. Thank you!

My local radiology center uses the Pylarify (18F PSMA-DCFPyL) tracer, but I'm thinking I might get a better result with the newer Illuccix (68Ga PSMA-11) tracer. Anyone have any recent experience with this?

Bone and CT scan done because my cheap insurance company denied the Pylarify until after the other 2. I don't mind the extra contrasts, barium or whatever hell else they gave me. I keep asking is it nuclear. Only the bone scan was.

I am waiting for approval from insurance to radiate 4 smalls spots only seen on Pylarify PET.

Kaiser denied my PET scan today because my PSA hasn't hit 2.0 above nadir yet on 2 consecutive tests. I am not surprised. I'll probably hit it in a few weeks when I get my next test so I guess its just a matter of time. I have to wonder what the benefit of conventional tests are at this time but I guess

(He had Pylarify scan plus MRI and CT in July 2022, and no lesions were yet seen in liver.) His disease has continued to progress since his Ac-225 treatment just 6 weeks ago, and he will receive no further treatments in Turkey. (By the way, he tolerated treatment well with no dry mouth!)
