ET with CALR, on warfarin and 45mcg
Pegasys
every 4 weeks. 3 vaccines, last one in November. Just when I started hoping I was going to remain one of the few... COVID got me.

Besremi Approval Saga - SUCCESS!
I have three doses of
Pegasys
left, which I will use up before starting the Besremi. I am planning to follow Dr. Mesa's recommendation for the starting dose for someone stable on 45mcg/week of
Pegasys
- 63mcg/every-other-week of Besremi. We will see how that goes.

Update 6.5
I have also experienced some itching with the Besremi (
Pegasys
too). Fortunately that has been well controlled with Claritin. If it appears I will not tolerate the 150mcg dose, the MPN-doc wants me to drop back to 125mcg and try that. He does feel the Besremi adverse effects are dose related.

Pegasys and insomnia
I’m now on a maintenance dose of
pegasys
. Started
pegasys
in 2016. In the Early years within 30mins of injecting I felt very tired and experienced a good deep sleep on my injection nights. Over the last year - not every time - maybe 50% of the injections I have difficulty sleeping.

Weight loss
Has anyone else on
Pegasys
lost weight. I seem to be losing 1lb a week and have now lost 11lbs in the last few months even though I’m eating the same amount as before.

COVID Positive
Per Johns Hopkins MPN Clinic nurse, PV +
Pegasys
does not make me high risk. Planning at this point to just ride it out. I am all stocked up and will just isolate at home. I was not planning to be part of the database of people with PV and COVID, but can now add to the data.

PEGASYS, help! Does the insomnia side effect ever subside?
Hi folks, started PEG 6 weeks ago and it will not allow me to sleep, it won’t even let me nap during the day. On top of my 14 hr a day profound fatigue it has been a complete misery and I also hate the feeling of sleeping pills in my system. Thinking I may have to quit it, the alternatives depress and

Haematology tests and outcome.
I know
Pegasys
is associated with diabetes and fatty liver. Has anyone else had a similar outcome?

Extreme reactions to vaccines
I have ET CalR mutation and inject
Pegasys
interferon once a week. I’ve never had any issues with vaccines previous to my diagnosis apart from mild side effects so I’m wondering if my extreme reaction might be to do with
Pegasys
?

Myelofibrosis diagnosis
He will be starting
Pegasys
interferon on Monday at a starting dose of 45 micrograms. I've joined this forum to get as much information as possible to help us know what to expect from people who are actually going through the same thing.

Besremi versus Pegasys
Further for
Pegasys
there are several different sites to which PEG can attach, I assume this is the "moieties" noted above: https://www.sciencedirect.com/topics/medicine-and-dentistry/peginterferon-alpha2a <<There are four main potential IFN-α2a pegylation sites, (for
Pegasys
) at lysine positions 31

Update 6.0
For now I am going to continue on
Pegasys
as I am responding so well to it. We plan to recheck my JAK2 mutant allele burden 3 years out from when I started the PEG, which will be May 2024.

Pegasys/ ET and HRT
I have ET, my platelets have been very high at over 1500 and I have taken up to 270mcg
Pegasys
weekly at one point and must say that was not the best. However I’m now stable on 90 weekly with platelets at 350-390. So I’m happy with that.

Besremi in Prague
Pegasys
, on the other hand, is not indicated for the treatment of this disease. In other words, Besremi is an on-label treatment in this case, while
Pegasys
is an off-label treatment.

Pegasys not working as well with Calr
At one of the recent webinars from the US, a haematologist mentioned that interferon doesn't work well with Calr mutation. Can anyone shed any light? Sadly that seems to be my experience, but I think my haematologist might not know about that research. Thanks Jo

Pegasys Update
For now, we are continuing the
Pegasys
at 90mcg/week (dosed at 180mcg bi-weekly). I will have another full blood work-up in 5 weeks and we'll see at that time if platelets remain stable we might be able to reduce the Peg to 67mcg/week.

Possible side effects of interferon?
I started
Pegasys
at 45mcg per week about 2 months ago and for the most part I haven’t noticed obvious side effects. However in the last few weeks I’ve noticed pain in my hips, I think it’s muscular rather than in the actual joint.

New Plots for Besremi & "Beneficial"
Interestingly there is no need to compare
Pegasys
since that is not FDA approved hence not "equivalent". Insurance rep looked for alternatives, expecting to find some. I told her you won't find any, she came back and said you're right.

Splenomegaly after taking Pegasys
I know
Pegasys
reduces spleen size, but does it by chance increase the size temporarily at initiation of treatment? I didn't have splenomegaly before, just wondering why I feel like I do after taking
Pegasys
. Not having an enlarged spleen was the one thing I had going for me!
