Fast PSA Rise
Has ADT for 6 months prior to Brachy and
IMRT
. PSA started to climb 10/2018. Had Axumin PET?CT scan which showed suspicious sites in prostate and hylar lymph node after a negative prostate biopsy. Really confusing tests etc. I assume that more ADT will be next. Any suggestions, advise etc?

Post treatment T
Gents, Completed EBRT/
IMRT
, bracyboost, and Lupron. Last Lupron was in early May; 3 mo duration. Nov 2019: PSA <.1 and T =7. So, your experience with return of T after radiation and ADT. I sure thought the T would be a tad higher 3 mos after Lupron. EdinBaltimore

Hello to all from someone with RP, ADT, and IMRT
I just wanted to introduce myself. I was diagnosed with prostate cancer in February of 2016 when my PSA rose to 10.6. I was GS 7 (4+3) and aggressive treatment was recommended. I chose RP and had surgery in April '16. The pathology report showed an extra-prostatic extension, but negative margins

ore details on degarelix (Firmagon)
After a single dose (120 mg) of degarelix to support my ST-ADT in conjunction with pelvic lymph node
IMRT
, my Testosterone level is castrate (>20 ng/dl). I added this as estradiol patches were slow to get there (T = 63 after 2 weeks).

Good news
Convinced the oncologist that I was oligometastatic That led to 42 sessions of
IMRT
in summer of 2018. Insurance did not pay, saying local treatment was not necessary for systemic disease. The University of Chicago has never asked me for a penny of what insurance did not pay.

Whole Pelvic IMRT at Mayo or Local?
I will be having
IMRT
to pelvic floor and all pelvic lymph nodes soon. But have decision to make. I live in NC, and spend winters at home in central FL.

Update on starting Estradiol Patches for RT ADT
This is for short-term ADT as adjuvant for
IMRT
of oligometastatic disease of the left pelvic lymphatics identified on Ga-PSMA scan at UCLA. One week in and 10 days more until the RT (60Gy/30fx) begins. I feel very good on this so far.

Finished HDR-BT and IMRT
So I guess
IMRT
killed a lot of lymphocytes over the last five weeks. At the end of
IMRT
, PSA is still <0.1 and no other SE's such as rectal burning, so the SpaceOar probably did its job.

ADT, Zytiga and Fitness
In between I've done the six cycles of Taxotere and 26 sessions of
IMRT
. Do you plateau on strength and endurance losses after a year or so of ADT, or does it keep getting worse? I used to do six sets of six pullups and now I'm down to three sets of three!

Update: Experience with Estradiol patches for ADT
So recently when I needed to start short-term ADT as adjuvant to pelvic lymph node radiation (
IMRT
with boost to nodes) I chose to try Estradiol patches.

Lupron side effects - recovery?
IMRT
+ brachyboost + 9 mos of Lupron here. Numerous side effects - from wt gain, hot flashes, emotional roller coaster to genital shrinkage, higher BP and bone density issues - poster child for side effects. Ugh.

State of the Art Radiation Therapy Equipment
The main difference between current forms of
IMRT
and SBRT, is not so much the precision but that SBRT just uses fewer doses of higher radiation. 2. All of these forms of radiation treatment, even proton therapy are going Stereotactic, with proton therapy bringing up the rear.
