Bicalutamide

I'll receive my first lupron injection after two weeks of bicalutamide, which I started Monday, followed by 2 more weeks of bicalutamide. Week 5 I begin daily xtandi and lupron injections every 3 months for an indefinite peroiod.

Treatment with just a CDK8/CDK19 inhibitor or with bicalutamide, a nonsteroidal androgen receptor antagonist commonly used in castration resistance, had only modest effects.

I started Bicalutamide 150mg 8/2/2022 until 23/2/2022 during 16 days and i did PSA test and the result was 0.38 it decreased from 1.2 It seems that the Testosterone is decreasing to the cancer cells .

It extended my use of bicalutamide from 1 year to 4 years. But I did each step to failure (ie BAT to failure, bical to failure) I think that was a mistake and you would probably get more mileage by stopping each step well short of failure.

But the only treatment he has been offered is bicalutamide monotherapy.

[I think back to a possible mis-judgement made when I failed bicalutamide, in only doing PSA every three months: my 9/2020 labs had PSA 3.8, ALP 108, in a nice decline from 6/2020... three months later, my 1/2021 labs had PSA 23.4, ALP 267.

She put me on bicalutamide (50MG), which I haven't started yet. My sleep has always been poor but lately, even before the diagnosis, has become very fragmented. I sleep in 1-2 hours intervals. I do get about 6-7 hours of sleep total, but not enough deep sleep.

My goal is to find the minimum Bicalutamide dose that will keep my PSA relatively low until this 4th Covid wave passes and can proceed with my next treatment.

PSA reduced to 1 and eventually gone down to 0.05 etc… He was on BICALUTAMIDE medicine for about 4 years and PSA was well under 4 for quite sometime. PSA started rising in 2019. His doctor didn’t change any medication but straight away went to Radiation.

Until 2019 he has been on intermittent hormonal therapy, especially with bicalutamide 50 and 2 injections of eligard until in 2019 the psa gave 15.

If there are mets we will probably try to zap them and after that continue with bicalutamide mono therapy (50 mg/day). Any thoughts on timing of PSMA scan (or any other scan) and when to resume therapy?

He was started on Lupron every 3 months, 30 days Bicalutamide, and now Zytiga 1000 mg plus Prednisone 10 mg. He just completed 2 weeks ago 55gy RT to prostate and lymph nodes plus 45gy to ribs mets, total 100 gy.

Urologist started me on Bicalutamide. Oncologist switch me to Eligard in Jan 21. PSA 2.42 in Jan. Staged as NO M1a IVB. Feb 8 & 11 CyberKnife, Prostate only. 22 Feb thru 26 Mar TrueBeam to both Prostate and four lymph nodes in upper abdomen. 22 Mar PSA .145.

Question is what about the bicalutamide (Casodex). Do I walk away from it for a while? Telecom with Uro next week.
