Question #2: Is there a drug-herbal interaction between green tea and palbociclib requiring separation of administration?
Although both palbociclib and EGCG can have anticancer effects, there does appear to be a drug-herbal interaction between these two agents. Separation of administration is unlikely to affect this interaction and the combination should be avoided due to decreased absorption of palbociclib caused by interaction with EGCG.
Background:
A recent 2019 study conducted in rats demonstrated decreased absorption of palbociclib both when given after 1 dose of EGCG and after 7 days of daily EGCG. In the rats that took EGCG for 7 days, they fasted for 12 hr before receiving palbociclib and decreased absorption of palbociclib was still seen, nearly identical to that seen after single-dose combination.
Maximum concentration (Cmax) of palbociclib, or the peak levels after a dose, was reduced by 11% after single dose and 14% after 7-day exposure to EGCG. Area under the curve (AUC), a measure of total exposure to palbociclib was reduced by 16% after single dose and 20% after 7-day exposure to EGCG. The time to maximum concentration (Tmax) and half-life (t1/2) were unchanged in both single-dose and 7-day EGCG exposure, indicating that elimination of palbociclib from the body is not affected by this combination. Therefore, decreased effectiveness of palbociclib would likely be a result of this drug-herbal interaction, but not increased toxicity from palbociclib.
