Green tea and Ibrance not compatible

I asked my onc pharmasist about possible drug interactions with green tea(I really only like to drink Iced green tea now!)

She just called me with this 2019 study...DAMN

Question #2: Is there a drug-herbal interaction between green tea and palbociclib requiring separation of administration?

Although both palbociclib and EGCG can have anticancer effects, there does appear to be a drug-herbal interaction between these two agents. Separation of administration is unlikely to affect this interaction and the combination should be avoided due to decreased absorption of palbociclib caused by interaction with EGCG.

Background:

A recent 2019 study conducted in rats demonstrated decreased absorption of palbociclib both when given after 1 dose of EGCG and after 7 days of daily EGCG. In the rats that took EGCG for 7 days, they fasted for 12 hr before receiving palbociclib and decreased absorption of palbociclib was still seen, nearly identical to that seen after single-dose combination.

Maximum concentration (Cmax) of palbociclib, or the peak levels after a dose, was reduced by 11% after single dose and 14% after 7-day exposure to EGCG. Area under the curve (AUC), a measure of total exposure to palbociclib was reduced by 16% after single dose and 20% after 7-day exposure to EGCG. The time to maximum concentration (Tmax) and half-life (t1/2) were unchanged in both single-dose and 7-day EGCG exposure, indicating that elimination of palbociclib from the body is not affected by this combination. Therefore, decreased effectiveness of palbociclib would likely be a result of this drug-herbal interaction, but not increased toxicity from palbociclib.

Well that’s a surprise about green tea...I’ve tried to force it down occasionally believing it would be good for me but I really hate the stuff

However lots of ladies do enjoy it...another deprivation?

Thanks for info..was only done on rats though so I wouldn’t panic...there are lots of trials on rats but they never reach the human trials

The article said even when it was taken 12 hrs before taking IBRANCE, there was 11% less effectiveness.!

I drink green tea and I was never told not to. I will ask. If this is only one article concerning rats than it isn’t definitive.

I wonder about the dosage of ECGC given to rats, and I wonder about giving it as an isolated compound rather than with all the other properties of green tea. I love matcha and will definitely look into this more. This from Wikipedia caught my interest too:

“When taken orally, EGCG has poor absorption even at daily intake equivalent to 8–16 cups of green tea, an amount causing adverse effects such as nausea or heartburn.[5] After consumption, EGCG blood levels peak within 1.7 hours.[6] The absorbed plasma half-life is ~5 hours,[6] but with majority of unchanged EGCG excreted into urine over 0 to 8 hours.[6]”

All things in moderation?

What does this this mean?

Agreed, green tea extract is a different thing altogether. I love green tea and used to drink about 3 large mugs a day. My onc reckons I can continue to enjoy my green tea but I have cut down to a cup occasionally which I am pretty sure will be OK.

Yeah, extract is not the same as the free wheeling green tea as a whole. I take studies on rats with a grain of salt. Once cup of brewed green tea is all I drink here & there. On a side note, I found a brand that doesn't taste like dirt...Numi Organic Gunpowder Green Tea.

Interesting! Take into consideration that it says it may decrease availability of Palbociclib.

My oncologist said green tea was okay as long as I wasn’t having a lot of it, he said one cup a day was fine. I usually only have it once a week now.

I actually was told by my nutritionist to avoid green tea while taking Ibrance. The Pfizer people who talk to you about Ibrance did not mention that. They just said to stay away from grapefruit, pomegranate and seville oranges. My doctor also did not mention to stay away from green tea. Luckily I have listed to the nutritionist and do not drink green tea any more.

I've been using green tea in my smoothies. Never again. Guess I'm back to alkaline water. What else do you drink and eat? I'm on Ibrance and anastrozole..just started two days ago. Hard to figure out with estrogen receptors cancer what to eat.

Someone serendipitously found this old thread….dam I love my green tea and I take Ibrance!!!I have a cup or two a day. Not sure what to make of the data…I’m sure my onc told me a few cups were fine….but warned me to avoid the usual suspects (grapefruit, pomegranate and Seville oranges) and to also avoid coffee ( which I’ve never drank anyway).

I’ll ask when I see her in about 10 days to get my scan results too.

Will let you know her verdict.

I know this is an old thread but I wanted to up date some information. I checked with my oncologist and she assured me that one or two cups a day is fine… but she mentioned to avoid extract and supplement forms and be moderate with consumption.Phew! So relieved as I love my tea!

Why avoid coffee ? That’s news to me.

Hi

not sure why to avoid the coffee… at the time I was on Kisqali or about to start it, as it was right at the first few oncology meetings. The Dr asked me if I’m a big coffee drinker and when I said "no never had a cup I my life" she said "good you should avoid it"… But it might of been more like the green tea and perhaps be more of a case of moderation…. But since I don’t drink it, I didn’t ask her to explain. So far following all advice given by this Dr I’m responding well and have good results.

She’s an advocate of having all things in moderation concerning diet…not to totally deny yourself the occasional cake etc... but she is also firmly against taking unprescribed supplements and herbal remedies, extracts etc.
