This looks like an inflamed hair follicle. But if you have had recent unprotected sex, worthwhile having a sti check at a clinic, including syphilis testing .

He says he has never had sex!!! I have had infected hair follicles and a penile cyst and none of them looked like that. He is better off having a consultation with a doctor rather than photographic guess work.

Ok apologies I missed that - then that rules out STIs and means this is dermatological. Not all inflamed hair follicles look the same. It should go away over time. .
