advise on Gabapentin

I have been using codeine to control my RLS , 30mg at 5pm, 60mg at 9:00pm and another 30mg at 1am , I’m usually awake around 4:30 which is still not enough sleep especially as I’m up at 6:30am , I’m a domestic within NHS, so a very physical job , it’s starting to take its toll again , I also can feel my mood dropping , I had an appointment with my consultant last week and Iv asked to try Gabapentin again ( didn’t work last time I tried , but I now know that i was augmenting from pramipexole) .

Dr has given me 100mg to take twice daily , so I take one at 5pm and another at 9pm along with the codine , first two nights there was a slight improvement in my sleep and was waking feeling rested , but the last couple of nights the RLS has been unbearable, I realise that the dose is not high enough yet , but has anyone else experienced RLS worsening with the Gabapentin , or do I need to stick this out , my intentions are to eventually come off the codeine.

The gabapentin shouldn't make RLS worse.

It takes 3 weeks before it is fully effective. After the 3 weeks increase it by 100 mg every 2 days until it controls your RLS. You should take your gabapentin all at 1 time.

Take it 1 to 2 hours before bedtime. If you need more than 600 mg take the extra 4 hours before bedtime as it is not as well absorbed above 600 mg. If you need more than 1200 mg, take the extra 6 hours before bedtime. If you take magnesium, even in a multivitamin, don't take it within 3 hours of the gabapentin as it reduces the absorption of the gabapentin. If you take calcium don't take it nor calcium-rich foods within 2 hours for the same reason.

Kiram. It sounds like you could be short of iron. Have you had a full panel blood test? If not please follow Sue’s advice on iron. My rls is still there but much easier and controlled with iron intake.

I’m climbed up the Gabapentin doses and then went onto Pregabalin. I’m on 450mg plus dihydrocodeine (30mg now 60mg MR)

Since then my sleep and RLS is massively better.

So I urge you to persevere. Follow Sue’s advice and also look at iron.

Hi Kiram, I’ve been on both meds but didn’t help. My consultant Jose Thomas gave me Methadone eventually after 5 months it was increased to 10mg x daily . One year later I still have the unpleasant sensations 24 hrs but the worst which stopped my sleeping lets me rest for 5hrs instead of 1.2 hrs. I have a reaction 6 hrs after the sugar which lasts for 26hrs only happens if I eat more than 3-5 mg sugar.

My wonderful consultant I’ve just seen he’s who’s looking after me for kidney cancer is looking into the reasons why this happens.

So sorry I can’t help, don’t think I can explain your predicament with the meds you mentioned.

Sending love and good luck - Patricia
