Could you just make this an extension to your preexisting intake diary?
I just used to write things down in a notebook.But there are various, often free, symptom trackers: just type Printable Symptom Tracker into your favourite search engine and a number will appear. I've not tried any of them.
There are also apps - again, I've not tried any - for example:
