Pregabalin and Pramipexole

Do you think it’s a case that these drugs effects different people in different ways and that one can augment on one and another on a different drug?

Different medicines do affect different people in different ways.

To some extent that is.

To some extent however, they have to affect everybody more or less the same way, otherwise there would be no point in prescribing them.

AS regards effectiveness, medicines are trialed and their effectiveness is judged on a statistical basis e.g the effectiveness of pramipexole is, I believe is 70% That for gabapentin I see from one study is slightly more.

When it comes to side effects, there are known side effects for each medicine, some are common side effects, some uncommon and some rare.

Nobody is likely to get a side effect that isn't listed, but not everybody gets the same ones or to the same degree.

When it comes to "augmentation" a recent study suggest that augmentation rate for pramipexole (0.5mg) is about 3 times that for gabapentin.

While these statistics can help predict the likelihood of pregabalin (gabapentin) or pramipexole working effectively, causing particular side effects or causing augmentation they do not predict how any individual will respond.

There is some evidence that if you've previously suffered dopaminergic augmentation

1) Gabapentin/pregabalin are less likely to be effective.

2) You're more likely to suffer augmentation again..

Incidentally, gabapentin is equally effective irrespective of whether anybody has taken a DA before, but only if they haven't had augmentation.

What is the recommended drug of choice if augmentation with DA drugs has occurred & Gabapentin is in ineffective.. ? I want to wean off my patch but have been on all the DA’s & GABA ... not sure what’s left?

The second line treatment for RLS if the first line treatments have failed (DAs or A2Ds) is an opioid.
