help with dissertation on stopping smoking

Hi everyone, I'm currently doing my dissertation on smoking and stopping smoking and I need people who either smoke or have recently quit to fill in a questionnaire that'll take around 10 minutes to complete. Any help would be greatly appreciated and would help us understand better the processes involved in quitting smoking and what is more helpful. Done it. Im not being funny but i found the ethnicity questions as of you was american eg 'african american' and 'pacific islander' etc. I thought it said liverpool. Is this british or american? Thank you
