Hello all,

Yesterday I was diagnosed with prostate cancer & would appreciate any guidance. Awaiting to be scheduled for PSMA PET Scan

PSA 18.987

MRI - 1 Lesion 9mm

Pathology: 4+3

A. Right Posterior Medial

Adenocarcinoma; Gleason Score 6(3+3); Grade Group 1; Tumor measures 8 mm in length; 44% of the core involved by tumor; 1 of 2 cores involved. Perineural invasion identified.

B. Right Posterior Lateral

Adenocarcinoma; Gleason Score 7(3+4); Gleason pattern 4 approximately 20%; Grade Group 2; Tumor measures 14 mm in length; 58% of the core involved by tumor; 2 of 2 cores involved. Perineural invasion identified.

C. Right Base

Adenocarcinoma; Gleason Score 7(3+4); Gleason pattern 4 approximately 20%; Grade Group 2; Tumor measures 2 mm in length; 8% of the core involved by tumor; 1 of 2 cores involved.

D. Right Anterior Medial

Benign prostate tissue.

E. Right Anterior Lateral

Benign prostate tissue.

F. Left Posterior Medial

Few atypical glands adjacent to High-Grade Prostatic Intraepithelial Neoplasia (HGPIN); Suspicious but insufficient for diagnosis of Malignancy.

G. Left Posterior Lateral

Focal High-grade prostatic intraepithelial neoplasia (HGPIN).

H. Left Base

Adenocarcinoma; Gleason Score 6(3+3); Grade Group 1; Tumor measures 1 mm in length; 4% of the core involved by tumor; 1 of 2 cores involved. Adjacent and separate High-Grade Prostatic Intraepithelial Neoplasia (HGPIN).

I. Left Anterior Medial

Benign prostate tissue.

J. Left Anterior Lateral

Benign prostate tissue.

K. ROI 1

Adenocarcinoma; Gleason Score 7(4+3); Gleason pattern 4 approximately 60%; Grade Group 3; Tumor measures 32 mm in length; 64% of the core involved by tumor; 5 of 5 cores involved. Perineural invasion identified

Ok. I was asking because you said “distant” spread. My MRI doesn’t show sign of spread but we will see once I have the PET SCAN. Can SBRT be used on local spread?

You will probably will reach 95 but won't realize that you did....They refer to that as mental fog......... I call it metal concrete......... God Bless to you too...

I sure understand where you're coming from. I started dealing with this shit at the age of 66 for 22 years. Don't fret with all the new meds and procedures you'll easily hit 88. But I warn you time goes fast and much faster as you get older.

Thank you John- Can’t say that I’d want to go through numerous treatments to live to 88. It seems like I’m going to be a lifetime slave to the medical system. It’s frustrating to say the least. Sorry just venting.

That's okay cause you'll be venting for a few years. All of us didn't ask for this shit....But just think of it as if you married it.... I predict that we will beat the beast in less than 10 years but sure enough we will be venting about something else that arises to torment us. Try to live a life of fun and a laughter and enjoy yourself. My hobby is humor.... and it has kept me sane for several years. So play the lotto and if you win, remember me.... 🤡🤡🤡
