Rising PSA

I had another MRI in January 2023 and showed the same thing as the other three, all about the same, Pirads-2. My concern is my rising PSA since August 2022 , 8.0, 10.5 and then 11.5 this week. I do have another surveillance biopsy next week but this rising PSA concerns me. Lab says they don't check Free PSA when PSA is over 10. Any insight is appreciated. Thank you.

No medications.

Then maybe it's time for Lupron or some other ADT drug.

No, I am on active surveillance. Trying to avoid treatment until and unless it is absolutely necessary. Thanks.

Don't wait too long. If you cancer is confined to the prostate, that's good. Don't wait for it to spread.

What is the size of your prostate?

4.1 x 3.3 x 4.8 cm. I was diagnosed with BPH and it has been about the same for the last three years.

Do you still have bladder outlet obstruction - that completely explains your high and rising PSA, as I've told you before.

I haven’t done anything about it so I’m sure I still do. If that was the cause I thought the psa would stabilize at some point.

No - obstruction will always cause high PSA.

Thanks a lot. Hopefully the surveillance biopsy on Monday will show the cancer hasn’t gotten worse.

I don’t know. I already know I have Gleason 3+3 cancer and bph. Last MRI was same as the 3 before that. I have been riding the bike for 3000-5000 miles a year for a long time. Maybe it’s catching up to me, maybe I’m just getting old.

I don’t want to treat it if I don’t need to. It would be great to just have radiation or removal and move on but I haven’t seen anyone do that without lifelong side effects.

It sounds like you're about where I was when I started treatment. A 4 month Lupron shot and 27 radiation treatments. I'm still able to function sexually as I did before treatment except for no semen output during orgasm. My PSA has been hanging around .25 the past 4 years. I would say go for it before it spreads.
