FW8 Prescription Form Help Please

Hi Everyone,

Received a call today to say that I have a UTI and realised that I hadn't sent my FW8 form.

I've completed parts 1 and 2. My midwife has completed part 3 but the GP name, address, postcode and stamp is missing. Do I need to complete this bit or is the midwife info enough?

Ive always just had midwife info never given it to the gp. Does it say shes a midwife under her signature... if you're not sure Give them a call.

I think it can be either. I asked a GP at my work to sign and stamp it as I wasn’t seeing my midwife for a while x
