Hi this is the beginning of my fifth year with PMR and I was down to 2 1/2 pred.
Three months ago I tore both Rotating cuffs in my shoulders, it was impossible to sleep as they seem to kick in as soon as I lie down. It has got to a point where I am dog tired but dread going to bed to face another sleepless night and pain. I have tried pills. they dont work. I had recently reduced to 2pg pred and wondered if part of my problems were that I was having a flare, so decided to up to 10pg and see what happened. I must say I do feel a little better, not the miracle that happened when I first took pred after being diagnosed, but also not so inflamed in my shoulders. So my question for the mighty sages there is this, how long should I stay at 10, will I improve any more with time, should I start to reduce again and if so by how much, All that said, I wish you all a very happy and hopefully healthy new year and once again many many thanks for sharing your time and your wisdom with us all.

Are the rotator cuff injuries diagnosed by MRI? Unusual for both to go at the same time - how did you do it? How long have you been on the 10mg dose? That is a bit more than we'd suggest for a flare but it should show if it IS a flare.
Agree with PMRpro - I think I'd want the RCs checked out, and how did you damage both at same time?.. and if ordinary painkillers aren't helping I'd be thinking PMR flare or possibly even GCA...
Usual advice for dealing with a flare is in this link-but you probably need to drop down a bit more slowly as it's a bigger increase than usually recommended. So maybe to 6mg for a week or so, then to 3mg rather than 2.5mg.
Hello Dorset Lady, I used very heavy loppers to cut down an old thick stemmed climbing rose high above my head, next morning I thought I had slept badly on my right side, a few days later I went to a physio who worked hard on both arms, next day I felt like I had been hit by a bus, my left arm started to play up soon after.
My doctor who is new to me gave me Solpadeine to take at night, it dulls the pain but I still have trouble sleeping, and I hate the fuzzy head that get with it. Thats when I thought maybe its a flare, last night I slept a little better, but have gone down with the mother of a head and chest cold so that too impaired sleep. I have read your comments, I will cut to six tonight and follow your instructions.
I should add that I have had this complaint for over four year s, and in four years I have never had anyone at my doctors surgery discuss it with me I have been left to just get on with it, any info I have had has come from you guys. I thank you for being there.
Hmmm - so who said rotator cuff? It is frequently offered as a dx when in fact it is PMR. And the loppers would do it - they are amongst our "DON'T use them" list.
Cold certainly wouldn’t have helped… and can commiserate with the loppers…. My GCA has long gone, but I have a RC issue same side as replacement- so loppers are now a no-no for me unless I’m VERY cautious…🙈
