I’m due to have major surgery soon. Currently I’m on 5mg pred. Should I increase the dosage for the time of the operation?

That’s something you need to discuss with your surgeon and anaesthetist- an infusion will probably be given during the surgery itself.. and do make sure that everyone involved knows you are on steroids.
Not off your own bat, no. You need to discuss it with both your surgeon and his anaethetist and your rheumy. The anaesthetist will provide a boost of steroid during surgery with the infusion they will have up and monitor you closely to be sure it is enough.
You do also need to ask the rheumy what they would do should the surgery trigger a flare post-op - it does happen and some surgeries are more likely to make a difference than others. It isn't a good look if you flare and can't move well enough to be able to get up and mobilise - especially if it is a joint replacement or other orthopaedic surgery where the post-op rehanb starts immediately after the surgery. And the chances of getting hold of the right person are variable - they might be on holiday or somewhere else and no-one is willing to make a decision! Get their instructions in writing and be sure your surgeon has signed it off or you might get ward staff having a hissy fit when you ask for more pred!
