Help with making good use of seeing Rheumatologist for first review

Some where I read some really good advise on how to make the best use of seeing a medical person be it GP or Rheumatologist by keeping focused on what you really want. There were a small number of bullet points that were so good but can I find the article. It was written I believe by a Doctor to help patients make best use of an appointment. One thing I am sure of is to make notes before I go but I just need the bullet points to help me. Many thanks for your thoughts.

Thank you 123-go yes it is exactly what I was looking for. I am not very good at using this forum but find it very helpful. It is not so easy as my Husband suffers from complex PTSD and is not keen on me spending what he sees as loads of time on the computer
