Ineffective oesophageal motility and excessive acid reflux

Hi all, I have a history limited cutaneous systemic sclerosis. I recently had 24 hour PH and Manometry test to check for oesophageal motility issues due to food getting stuck when I eat and chocking episodes and to see the severity of the reflux I’m having. Despite 2 years of a combination of omeprozole/Esomprozole and ranitidine for symptoms the tests revealed I have ineffective motility at least half the time and despite what I thought to have been a good day in terms of reflux during the test it shows I have excessive acid reflux having 73 episodes in 24 hours. My question is given that I’m already taking meds to help, what other treatments are available? Anyone here needed surgery? Very concerned it’s going to get worse.

Any help would be appreciated

I have exactly the same symptoms and in this last twelve months have had two rounds of ph acid tests, bile malabsorption test, gastric emptying test, testing for sclederma, barium swallow and probably others I have lost sight of, oh yes, a bravo ph capsule to further check reflux plus an endoscopy and biopsies.

I have been diagnosed with slow gastric emptying, severe acid reflux, hiatus hernia inflammation at the junction between my Oesophagus and stomach and ulcers there too. Been offered a Fundoplication op but been told as my motility is so bad and almost total peristalsis that I will probably be worse off with other symptoms like inability to swallow and severe bloating which most likely would make me more Poorly and not recommended but it's my choice.

I am in agony with pain ongoing and nothing improves it meds wise, I am stuck and at a loss to know what to do. I cannot take ppis as I have another condition where those meds are not allowed. It's beyond being able to manage so I sympathise with you and hope someone somewhere can offer you some treatment that works, I am at a loss to know where to turn now. Do you have access to a pain management programme at your hospital I have been offered one to attend. Best wishes I so know how you are feeling. X

For your education, question to be asked to the GP at first instance would be,

Are the medications for symptoms management or fixing the root cause issue?(GORD)

Can you get referral to see the GI consultant to fix the root cause issue?
