Acute Trochanter Hip Bursitis

Anyone ? I have high active RA & diagnosed with the trochanter bursitis a few days ago. I can’t begin PY for another 3d.

If you’ve had this, how long did it take to resolve. I’m in such pain right now. It’s been since Th. I am almost 4d into it. The pain worse at night and first thing in the am pain is excruciating when I have it no fever or redness etc X-rays take me in office said all in hip look good I ice area as much as I’m able the pn is more in the upper/side part of the thigh and some in the hip

Anyone know if this would have anything to do with me taking Humira injections ? Been on Humira since sept 4, 2021.

I’ve had such issues with all RA treatment so far. I had pericarditis in upper chest L side, gout in R big toe and now this. All have come on sudden. All but pericarditis, it is gone.

I personally want to get off Humira. Anyone know if weening off Humira is necessary and if so, how does one do that if the injections are bi-monthly?!?! It’s not a pill so I can’t take less of it ?!

I have what was diagnosed in the UK as Greater Trochanteric Bursitis (right). It comes & goes but woe betide anyone who touches near my hip when it's angry.

Physio helped following diagnosis but now when it flares up I do the exercises given to do at home. Thankfully it doesn’t bother me often anymore. I empathise with you if yours is flaring at the mo.

I'm not on Humira so unable to help there I’m afraid but your Rheumy should be able to throw some light on that or possible other causes & if it's time to look at alternatives.

Not sure if this is any help but I hope you find a solution soon. One thing the Physio told me was my weight couldn't have been connected, it often is apparently. The only treatment recommended was an increase in my NSAID which I chose not to do, & using an ice pack which I did try but I don't respond well to cold, heat often helps but not for GTB.

Thanks for your reply. NSAIDs were mentioned. Doesn’t touch this pain. I have a script for PT but don’t start till next Wed. Cold helps for a very short time. Heat I’m sure would feel lovely but they said cold. I’m 115lbs. This came out of no where. I’m switching Rheumatologist New one appt in Feb. ortho clinic said X-rays hip looked good. I have to keep working. Sitting is horrible and standing hard to walk and going downstairs. Not good either. I have taker. Vicodin (use sparingly ) to tie me over but I need this inflammation and mostly pain too to go down. I will get it figured out. Thx again for your reply

The increase in NSAID was intended to reduce the inflammation, but that would in turn reduce the pain. Either way I didn’t increase it.

Yours isn’t as a result of carrying more weight then either. Do you have muscle wastage at all? That can be another cause, but your Physiotherapist will be able to determine that & work on exercises to help build them up. Actually I've just found this blogs.bmj.com/bjsm/2021/10/...

Ive had GTB for years , the only thing that has really helped is a steroid injection which gives relief for a while , I usually don’t get this until pain is really severe . I have been referred for physio again and i have an appointment in January but im not hopeful . I can’t lie on the affected side as increases pain , and I sleep with a pillow between my knees .

I've had this twice (pre RA diagnosis but think its all related) you have my absolute sympathy.

Just the weight of my leg would make me swear like a sailor, bloody hurts !

Both bouts lasted about 8 weeks for me (sorry), I found it so hard to walk, best thing was rest.

I told rhematology I had previously had this and they said if I had it again they would inject with steroid (even thought of someone doing this makes me wince) . At the time when I had it GP gave me naproxen and strong pain killers, also had 1 physio appointment which wasn't the best but 🤷‍♀️

Hope it goes quickly.

If you feel Humira isn't working for you I would definitely ask what your options are medication wise with your rhematology dept

Yes definitely going to try and not take Humira anymore. I’m between docs so it’s a weird place to be with who I ask about this. I had a cortisone injection last TH. It’s not done much at all bc Pain is almost identical to how it was when I woke TH am. This pain is terrible. Not getting sleep at night. Standing hurts sitting is worse. Doing my best to hang in there. Adding a. It more ibuprofen to my day. Will see if that makes a difference. Thx for everyone’s replies.

It can go overnight..or hang around for weeks.My Bursitis has migrated to my elbow..much more controllable.

I have bi lateral trochanter bursitis worse on left hip . I had it for about 20 years , I live to have a steroid injection it’s the only thing that calms it down. . Putting ice on it makes it worse , I use hot water bottles . I find I have to keep moving even if it’s a little walk when it’s flaring or I cease up . When I get in the car I always have the heated seat on so I can cope with sitting for 20 minutes . I’ve been told they’re going to refer me to orthopaedic hip consultant . Each year there seems to be a little less of me . I can’t really offer advice just a hug and to let you know I feel your pain 🤗

I’ve had it quite a few times. I usually have steroid injections which do help a lot. It’s strange because it seems to be self limiting. Last time it was agony. I couldn’t walk then suddenly it just went away. Unfortunately it returned a few months later. I’ve had it In both hips but usually now it always the same one. Both hips are always tender on the outside. As Heels said if anyone touches my hip it’s really painful.

Hi my lovely. I've had the same for two years now. I've had three ultrasound guided steroid injection into the area. Gave me relief for about 4-6 months. I take amitriptyine at night to help me sleep and pain as otherwise I'd be a walking 🧟‍♀️

Hope it gets more settled for you. I've tried gels and ice packs but nothing helps. Walking, sitting and sleeping is a torture but I can't take pain meds at work so suffer during day. Please ask your consultant or clinic nurse for some pain relief.

Thank you for asking the question and thanks to all who have replied so far. I have suffered with Bilateral Trochanteric Bursitis for over 2 years and I feel so much better this morning reading that I am not alone.Apart from pain relief (paracetamol, ibuprofen, tramadol and Amitriptyline for overnight), I was surprised that there was little or no treatment for the condition. I have had steroid injections into the bursae but in order to have these, I have to go through excruciating pain whilst the rheumy is identifying the site.

I was recently told that the the bursae are inflamed because of osteophytes in my knees making my gait compensate for the pain there. This was news to me.

I am now in the process of getting hydrotherapy to help solve the issue with the knees/hips.

I have been on sulfasalazine along with Mtx in the past but now just the latter.

Probably not very helpful to the OP, sorry, but I'm pleased to know of others' experiences.

Don’t cross your legs

Having mine injected regularly improves the pain

I’ve had Trochanteric bursitis for many years & had many steroid injections into hips. I find it difficult to lie on either side. Just after lockdown it was the worst it has ever been & I couldn’t put my weight through my right hip so limped for about 6 months till I could get seen face to face & had a further steroid injection which in February & has helped so much. The only thing that helps a bit is sleeping with a heat pad around my hip area or lower back. X

I have RA and I’ve had GTB actually got it at the moment I did get a steroid injection to the thigh a few months ago It helped for a couple of weeks but came back with a vengeance I can’t bear to lie on that side and it’s also in my hip I do the stretching exercises but not making any difference I do wish you well and that you get some ease, but I do know what your going through

I’ve got mild trochantic burstitis at the moment. It showed up on an MRI I had scan back at the beginning of August. I’ve got a sacral insufficiency fracture and also a lot of problems in my lumbar area but the bursitis pain was a worry for me.

To be honest it was a relief to find that because I was beginning to think my hip was going to pop out if it’s socket or disintegrate, so being told I had burstitis explained things and I was glad it was only showing as ‘mild’

I had it many years ago when I was much younger and I’m sure it was much more painful then, this latest bout started shortly after I started taking bisphosphonates (alendronic acid) for osteoporosis. Between that and a lot of other things I stopped the alendronic acid after four miserable months and the pain has gradually gone down.

I have a chill pillow (I bought in Tesco but they are probably available on Amazon) I got it for when it was hot in summer and when my hip was at its most painful I found that if I stuck that under my hip and lay on it it kept my hip nicely chilled. In fact I bought a second chill pillow to keep in reserve for when the first one warmed up.

I’m taking hydroxychloriquine for my inflammatory arthritis. I’m sure it was the alendronic acid that did it for me because since I stopped it I hardly feel my hip.

I do Pilates three times a week with a teacher who specialises in people with osteoporosis and problems of that sort and who has read my scan reports so she knows exactly what’s going on - I think the gentle stretching helps keep me mobile.

I just take paracetamol - the rheumy I saw kept saying she couldn’t npbelieve I wasn’t in a lot of pain and that she couldn’t believe that I was controlling it with paracetamol but I’d worked my way through other painkillers which nearly finished me off.

I took a lot of hot baths with magnesium flakes in them, the warmth helped I also used a lot of volterol gel in that area for a while but I haven’t used anything for a while. I’d say it took about three months to really settle. Burstitis pain was something I found hard to shift.

I had it a while ago. Since starting on Humira biosimilar it has got worse. Heat, anti inflammatory drugs and physio have helped me.

I have had this in the past. It was helped by steroid injections (may take a week or more to work). I have psoriatic arthritis. I also have a disease called DISH (Diffuse idiopathic skeletal hyperistosis). Lots of bone Spurs etc and tendons turning to bone like structures where they join on to bones. I think this is what caused mine. I find heat works far better than cold and perhaps a cushion under the buttock on the effected side when sitting. Keep moving though. And don’t skimp on pain relief! I found ice packs, stretching, gentle excercise, physio and CBD balm helpful.
