I have swelling in my knee and elbow joints that i can see and feel. Its obvious to me because it causes me pain and as not always been there. If someone else looked at it with an untrained eye they might not think anything was wrong. Its not like the swelling caused say by a broken bone which is normally quite an obvious protruding swelling.

I recently changed rheumatologists for a whole load of reasons. One reason was that my old rheumy only took any notice of whopping great swellings and I was sure that I was developing some less obvious ones in troublesome joints. My new rheumy is great - he examines me thoroughly and sees swelling where I see swelling. So in my experience inflammatory arthritis can produce different size swellings. As you imply, the important thing is to be examined by a doctor with a trained eye & I'm sure most rheumys have that.
The best thing to do is see a doctor. Who will do relevant tests and refer you as necessary. Not too sure what answer you want to your question? Sorry.
I am seeing doctor on monday andgoing to see private consultent if at all possible, not prepared to go on waiting list.

Sometimes with swelling you will get warmth around the effected area, also sometimes you will get a redness on the skin, the swelling will generally feel firm
