I had an attack of IGA about 11 years ago, My GFR went to 60 and hasn’t changed. I have no other markers e.g protein is fine, ACR and I don’t have any blood in my urine (micro or macro).
I’m not naive, I know there’s a potential time bomb in there. But, has anyone here had an acute attack? When was your last one and how much did the GFR drop. I’m 56. My specialist gives me absolutely no indication of the future and said it could be 20 years between attacks but, pretty much said it’s coming. He also is recommending SGLT2s as a precaution and frankly Im thinking he’s curious about the impact.

I don't have the disease, so can't comment on recurrence from experience. But since they don't know what causes IgA nephropathy, I can see why your doctor really can't tell when the next attack will happen. I would personally take the SGLT2 inhibitor if I where in your place (if tolerable), the drug has been shown to protect the kidneys in multiple trials (but not with IgA nephropathy that I know of).
Thanks Bumblebee. I think SGLT2 has has been identified as a new treatment targeted at IGA due to the hyper-filtration efficacy. Cheers
As my function isn’t slowly reducing (acute attack) I’m thinking the SGLT2 may do 2 things a) support the kidney performance in the event i do enter into reduced kidney function b) possibly assist if there is another attack (unsure how, possibly open the filtering system and allowing protein to pass. I don’t know)
As most SGLT2 people is prescribed to patients with lower function in Australia (public health funding) I think my specialist is curious to see what occurs.
