Need advice on what to do

Hi, I'm new here and seeking for advice.

Last month, I had blood and urine test, my creatinine went up, from 0.92 (taken Aug 21 2018) to 1.38 (Jan 22 2020), 17 months apart. And my gfr went down, from 98 to 60. All other tests were normal, potassium, sugar, sodium, etc. I am 30 years old, 140lbs and have mild sleep apnea and pre-hypertension.

Prior to the testing, I went on a trip 3 times in 2019. I didnt care what I was eating because I knew everything was ok.

After getting the result, my doctor told me to exercise and stay away from salty food. Right now, I have been eating veggies and fruits that are low in potassium and protein. Also, I have not eaten pork and beef. I dont have the energy to do exercise because of sleep apnea effect to my body. And by the way, my bp went down to 121/77. Before my diet, it was 145/90.

1) Should I be worried? Will my gfr stay, go down, or go back up?

2) Should I continue low potassium/protein diet? Although my doctor didnt advice me to do so.

3) Is it normal for gfr to go down that much within a year, because of eating unhealthy food?

HI Gfr,

So many questions and they should be something you are asking your doctor. But, you say you have apnea. Are you addressing that? Not sleeping and having apnea is very bad for the body. It can put a huge stress on your heart and cardiovascular system, often resulting in high blood pressure.

So lets take this one at time. Get a sleep study done and see if you need to be put on a CPAP machine. Made a big change in my life.

You have a pretty normal BP with the 127/77. But you need more than one to determine how your BP runs. Can you get your BP read more often?

Then have your doctor run another test soon to see if your GFR is still where it is currently.

Yes eating better will help. But a drop of that much.... I do not think that is just from the food. My opinion. I think you need to have more tests done to see why your GFR dropped 38 points. Make sure you are hydrated before your next labs.

The fact that you say you do not have enough energy to exercise is another area of concern. I would again, have labs drawn to see where your blood levels are for anemia.

There may be nothing more but if you do not research it out, you will never know. A good diet will help, but it may not solve everything. Find out the cause of the drop.

Hi Bassetmommer, thanks for replying.

I already had a sleep study done, I have mild sleep apnea and they gave me CPAP machine. BUT, I have trouble using it because of my clogged nose, so an allergy doctor prescribed me nasal spray/steroid to lessen inflamation of my nasal passage. Im gonna start with the spray sometime nextweek. Just got my wisdom teeth removed.

My bp has been consistent, 120ish over 77. We have a machine at home, I check it everyday, so far so good with my bp.

You've mentioned being hydrated before the test. I think I was dehydrated (I had a habit of not drinking water often) when I took the test and I ate very salty shredded pork prior to that. Please tell me those 2 things affected the results =(

I've had complete blood count as well along with urine test, my doctor didnt mention about anemia. white and red blood cells were normal.

There are different cpap masks you can use. My husband likes the nasal pillows but there is one that covers the nose and a larger one that covers nose and mouth. See if you could try a different one.

Hi Melissa, I tried both (nasal pillow and full face mask) and neither worked. I would wake up in the middle of the night gasping for air. I will have the cpap company change the air pressure and see if that helps. Will update you about that. I just want to clear my clogged nose and be able to breathe good =(

Yes they may want you to be re-evaluated to see if the settings need changing. Good luck and keep me posted.

HI,

Two things...make sure the nasal spray he gave you is not harmful for the kidney. Steroids often are. The fact that you are too tired to exercise at all..... and at your age is concerning. And yes, being dehydrated and eating salty pork is not a good thing prior to a test. But to drop 38 points in a year is also concerning. Have another series of tests done after a couple of months on your improved diet and see what the results are.

Hi Bassetmommer, I just had a bloodtest done this morning, and I just got the result now.

Creatinine is 0.83

GFR is 109

Any opinion to what happened? Like the results dropped and went back up. Thanks!

We can't give medical advice but if the things you are doing are helping your BP then probably not a bad idea to stop. You should always do as your doctor advises unless you feel it is bad advice then you should seek a new doctor. Is your doctor treating your sleep apnea. If not seek a doctor who will. Exercise is very important.

Hi Melissa, just got today's bloodtest result.

Creatinine = 0.83

GFR = 109

Still cant believe the results.
