DVLA Form

My husband contacted the DVLA and told them he’s got a type of Parkinsonism called MSA. They have sent him a form for Parkinson’s disease and another one which doesn’t seem to apply at all. So is the Parkinson’s disease form the right one to fill in?

Hi there. Our consultant completed our forms and sent them off.We noticed DVLA but form went straight to consultant as I recall.

The DVLA sent the forms to us unfortunately. It would be much simpler if he had Parkinson’s disease as opposed to Parkinsonism

Hello I can't recall what we did, its maybe worth asking the MSA Trust. (If there isn't a category for MSA maybe they could be asking for one). Our recent experience is that the DVLA is subject to industrial action just now and the service is overwhelmed. Kx
