Ruxo effective dosage levels

I’ve read that Ruxo dosage needs to be 20-25mg x 2 daily in order to reduce the risk of disease progression.

Does everyone agree? Some people seem to do well on 10mg x 2?

And I assume that just reducing the % JAK2 does not necessarily mean that the Ruxo is suppressing the all important clonal growth that leads to progression/more mutations.

ie it only has a chance to work at the clonal level if at a high dose level?

I’ve started on 10 mg x 2 and hoping this doesn’t overly suppress my bloods so that I can up the dosage if the above in true.

I started out at 10 mg twice a day in June 2023, but by March 2024, it started to fail and then increased it to 15 mg twice a day. It shrunk my spleen at the beginning and since it increased to 15 mg per day I’ve had no problems at all with my spleen. In fact, most of symptoms I had before went away.

I have never seen or heard that such doses are needed to reduce the risk of progression, if you have that handy it would be interesting to read it?

But if for example if 10 +10 optimises your counts and symptoms I would assume 20 or 25 BID would lower your counts too much. Dr V said to me 60 need more than 10+10mg and 10% need less. He said at the time first control reds then whites then platelets and of course symptoms.

That was 7 years ago so maybe things have changed but in my discussions with experts and conferences I havnt heard that big doses are best to reduce risk of progression and maybe more importantly I havnt heard that any dose of Rux will reduce risk of progression. In my case on 20+20 my AB is allegedly 1.08% and allegedly my BMB in 2024 was better than 2017 BMB. I have only been on 20+20 since around 2022 prior to that it was a start at 2.5mg BID in 2017 and slowly increases to 12.5 +10 by say 2020 , stayed at that for a couple of years and increased slowly to 20+20 by say 2022, now 22.5+20.

Ie I havnt been on high dose all along.
