This is s helpful short guide put out by Macmillan. UK based- but England, Wales, Scotland and Northern Ireland all have different access to meds (as I found out the hard way).

The Australian Leukaemia Foundation put up a series of optimal care guidelines for patients and for practitioners. Written to be accessible, the hive good, common sense advice. The MPN patient one is here: cancer.org.au/cancercaregui...

the MPN Voice website is also very good info, I think written by Clair Harrison and Co
