Please could somebody explain the difference between Carl positive & Carl negative and what does the Carl mutation represent.I attach a picture from my porch..

There are three driver mutations that are the cause of MPNs. In order of frequency - Janus Kinase 2 (JAK2), Calreticulin (CALR), Myeloproliferative Leukemia virus oncogene (MPL). All three of these mutations occur in hematopoietic stem cells and cause the upregulation of the JAK-STAT pathway. In essence, the "on-switch" for blood cell production is always on.

When assessing for a MPN, it is now common to check for all three driver mutations. The CALR mutation can cause ET or MF or in exceedingly rare cases is reported with PV.
