Because I am currently in China, and the only Jak STAT inhibitor available on the market in China is Ruxolitinib. I have been taking this drug for 5 years and have clearly shown intolerance, with low blood red and low platelets. In order to continue taking Ruxolitinib, I took hormones for a long time before, but then side effects also came along. So my attending doctor asked me to try Roxadustat and Revolade these two drugs. Roxadustat can improve hemoglobin and Revolade can improve platelets, and effects are very good for me.
But I still worry about the side effects of these 2 drugs, Roxadustat and Revolade. They may cause Exacerbation of bone marrow fibrosis. because these 2 drugs like EPO and TPO. I don't understand the drug mechanism. My attending physician is also an attempt on me.
So, I ask for help here.

I am not familiar with these two drugs. Here is some basic information.
Roxadustat is a hypoxia-inducible factor prolyl hydroxylase inhibitor that promotes erythropoiesis by increasing endogenous EPO levels by stabilization of hypoxia-inducible transcription factor (HIF) and, by modulation of hepcidin levels, improves iron regulation. sciencedirect.com/topics/me.... Note that roxadustat is not FDA approved in the USA. It was found to be ineffective in MDS treatment. fiercepharma.com/pharma/end....
REVOLADE (Eltrombopag) Unlike native thrombopoietin which binds to the extracellular domain of the thrombopoietin receptor, eltrombopag selectively binds to the trans-membrane domain of the receptor. It stimulates megakaryocytopoiesis through the Janus kinase/signal transducer and activator of transcription (JAK/STAT) signaling pathway. ncbi.nlm.nih.gov/pmc/articl....
I certainly do not have the expertise to offer any meaningful advice on these medications. The only suggestion I can make is to explore each of your options and then review with your hematologist to see what is available. Here are two videos that may be helpful.
