Gout medication advice please

Good Evening

Hope you are all well

I ama 50 year old lady

My uric acid has been high since 2020 but when I asked my old doctor he said it was nothing to worry about

I moved house and saw my new doctor sept 2022 and said I had gout and needed treatment

I have osteoarthritis in my knees, elbows, hands and I thought the terrible pain in my feet was my arthritis

She put me on allurpurin tablets 100 mg taken once a day

These gave me a horrendous upset tummy with cramps in my stomach and my poop was like water after 1 week of starting the tablets

Called my doctor and she said to stop taking them straight away which I have and my tummy is settling down

Can I treat my gout problem by diet and not have to take tablets from the doctor or is there anything better than the tablets which they prescribed me

Just don't want the tablets causing me problems elsewhere in my body

Thanks for any help and advice you could offer me greatly appreciated

Kindest regards

Janet

Are you taking any medications for the MPN? Some meds can cause renal impairment, leading to gout.

Hope you are well

Thank you so much for taking the time to reply to me greatly appreciated

I think my problem is Psoriatic arthritis not gout

My symptoms match the PA ones not the gout

Had high uric acid since 2020 and I have never had flare ups redness on my feet, warm to touch or swelling

My toes are deformed by herberdens nodes like a lump on my big toe

Trying to get another appointment to see my doctor but difficult to get to see someone

Thanks for your kind reply

Have a lovely day and evening

Kindest regards

Hello Janet , , ive had a number of bouts of gout varying in severity. Its my understanding that Allopurinol is a long term preventative taken daily which is unlikely to calm an attack if it flares up. I used to take Diclofenec which worked well but it was taken off market i think in tablet form. Also given Colchicine but i couldnt get off the loo and made me poorly, but might work for you.

I would want my Doc to not be so dismissive about my Uric acid level and get to the root cause instead of treating what could be a symptom. Have you been advised as to what might raise yours.

Im unsure if a change of diet will help greatly but there are a few natural remedies such as cherry juice / cherries and a host of the usual supplements which claim to help but without checking them out could be a waste of money.

I hope you get some resolution and please ask your new doc about it because it might be linked to your Osteo arthritis.

Regards - Chris

Good Evening Chris

Hope you are well

Thank you so much for your kind reply your help and advice is greatly appreciated 🙂

The doctor said I have had the gout since 2020 but was never given anything for it. I thought all my pain was just down to my osteoarthritis which I have in my elbows hands and knees and now my feet. I have haberdens nodes on my toes and fingers.

I am drinking cherry juice and lots of water and eating an anti inflammatory Mediterranean diet

The gout is definitely linked to my arthritis. But like you say doctors don't get to the cause

Have you had an x-ray on your feet

Hopefully will feel better soon

Thanks for taking the time to reply to me

Hope you don't get any flare ups because it is painful

Enjoy the rest of your weekend

Kindest regards

Janet

Its been a good while since my last flare up thankfully because its generally in my toe joints and extremely painful so i dont miss it. Mine was linked to my MPN. Not had any xrays on my feet , , about the only place not imaged or scanned but definitely gout , , , you certainly knowvabout it when you have it.

I hope your diet helps to reduce your Uric acid level and you stay clear because it sounds as though you have much to contend with in your daily life as it is.

Im just chilling with a couple of beers so enjoying simple pleasures, , and cheaper than going out which is rapidly getting beyond my limited means .

Take care

If you don't treat the Gout [Uric Acid levels. you'll end up in bigger problems in time to come.

I had high uric acid levels due to INF injections however well controlled and normal now for several years.

Try the following drug ( Adenuric) or the other name ( Febuxostat) which is more effective than Allopurinol and less side effects. However we are all different when it comes to side effects from medications.

If untreated, high uric acid levels may eventually lead to permanent bone, joint and tissue damage, kidney disease and heart disease. Research has also shown a link between high uric acid levels and type 2 diabetes, high blood pressure, and fatty liver disease.

Gout occurs when urate crystals accumulate in your joint, causing the inflammation and intense pain of a gout attack. Urate crystals can form when you have high levels of uric acid in your blood. Your body produces uric acid when it breaks down purines — substances that are found naturally in your body.

I assume you have had a Bone Density test carried out. If you haven't please get it done asap as its of paramount importance.

Forget X-rays... minimum CT Scans or better still an MRI.

Hang in there you'll control it ...just takes a little time and the right medication.

All the very best.
