Operation safe with ET jac2 pos ?

Hi all I’m having to have a what I hope is a minor op for a removal of a cyst but it needs to be done under a general anaesthetic I’m so scared I have pre op assessment on 12 th sept it’s at a private hospital many thanks Holly

Hi Holly, I’ve got PV and been through about 6 ops with general anaesthetic for each one since I was diagnosed. I haven’t had any problems but I always have my bloods tested just before op to make sure all is well and they have special boot things in theatre which massage your calves to keep blood flowing. You will have the joys of those sexy stockings which I hate but I know are necessary - you will be absolutely fine!

My knee replacement was a longer op so my haem and Orthopaedic consultant spoke to one another prior to the op. They really do take good care of you - you are wired up to heart, bp, oxygen monitors.

Bless you Aime you have been through it eh I also have a mitral valve issue but they will know about this I will have bloods done on 12 th and the usual mrsa check you take care my friend best wishes Holly x

I have answered your reply to me,I have a stent too ,I forgot to say,yes as Aime says ,you have the sexy stockings to look forward to.Mine being French had a lacy frill at the top!!!My nurse (male)who does my bloods monthly and has looked after me for 8 yrs showed me how to put the things on,much hilarity from my watching hubby,who never got the hang of it!!!! You will be fine,Honestly! Sally (again)!

Hi Sally, lucky you getting stockings with lace!😸😸. Yes it is entertainment watching someone struggle with these wondrous inventions. They are really difficult to get into post stomach or knee or hip ops. I can remember sweating buckets and being totally wiped after getting them on. My hubby was not the most gentle and tried to use more brute force rather then technique.xx😹😹

Thank you Holly, let us know how you get on but I’m always comforted by the care you get and also the monitoring of vitals plus when you are asleep the anaesthetist is solely looking after you and can control your breathing, etc. They laugh at me now (I’m lucky to have private healthcare too) because I can tell them the procedure pre op - I’ve got it off by heart! At least I give them something to be amused about!

Ps something I always ask for is anti sickness meds at the same time as the anaesthetic and I’m fine post op.

Hi.

How the surgery proceeds will depend in part on how well controlled your ET is. I had to have a similar op a couple of years back. (At a major private London hospital.) My counts were off on a frolic at the time but the op couldn’t wait. My haem (Prof Harrison) had stern words with the surgeon! Time under anaesthetic was kept to a minimum and I was to be treated as if high risk. I could only have the essential parts of the op carried out - which was irritating. Anyhow it all went fine. I had to stop aspirin ahead of surgery. So I guess my advice is, check with your haem first to see how they want to handle it. My experience has been that clinical staff generally can have very little experience of MPNs and you may need to take the lead. Hope all goes well.

I’ve had three lots of surgery with ET Jak2+, all with a general anaesthetic ,two of them lasting for four hours . My surgeon always liaised with my haematologist . The hydroxy continued but my aspirin was stopped beforehand . I then self administered anticoagulant injections afterwards for ten days . You’ll have the best possible care I’m sure 👍😊

I’m jak2 neg with ET and, over the decades it’s been diagnosed, I’ve have over 10 general anaesthetics. Don’t worry.... they know what they are doing. Good luck! 😀

Hi there Holly,

I worried too but unnecessarily. I was amazed that they need to give me a general for just a removal of a cyst on my back (in May) but they did and they did take into account my PV (Jak2+) so I think they gave my an extra shot of something (Heparin I think) and gave me stockings but I recovered really quickly because I wasn't under for long. No sickness and they gave me a bag drugs for pain relief which I didn't need at all. In fact, quite enjoyed having some 'time off' ! lying on crisp white sheets and being waited on hand and foot for the afternoon!!!!! Vanessa

Hi Holly, I had a total knee replacement, so had a general anaesthetic, also they put some kind of floating boots on me to make sure that I did not get any clots. Like you had bloods done before hand, came off my medication a couple of days before op, and returned to medication a couple of days afterwards, they did my bloods every day to make sure all was OK.

You just have to make sure they know your health history which I am sure you will, and seek any other advice you can give them from your Haematologist.

I had a bunion done before Et diagnosed. They requested that I stop taking clopidogrel 10 days before my op. Even though my platelets were over 500!!

As long as they are aware of your condition and consult with your haemo, you will be fine.

I had a heart procedure (AV Node ablation) some weeks ago. The cardiac doctor contacted my haeamtology consultant beforehand because as the latter had told me: "your platelets won't behave like everyone elses". I have ET. It would be very wise to get your doctor to make contact too. I wouldn't think it would matter that you are going to a private hospital - they all have the same duty of care for their patients. When I had a pacemaker put in 10 years ago I didn't think to speak to haematology and I ended up being very, very bruised afterwards because of the increased platelets. I also know that ANY time I end up in hospital I have to ask for the on-call haematologist to be contacted. They are the only people who understand our blood condiitions. Good luck with the op.
