Are kitchen hygiene standards acceptable or should more be done?

Kitchen hygiene standards usually seem ok in most big chains, I think. There are some take-always that I wouldn’t touch even if they did anything I could eat. The hygiene rating seems a good idea, but not everywhere appears to display them, it might be a helpful if they did.
