Hi folks. New here. Long post alert.

I was diagnosed with Hi IOP three years ago and at the time was prescribed latanoprost which reduce my pressures from mid thirties to low 20s

I had no follow-up during Covid then at the beginning of June in 2021 I suffered from a cute angle closure and had pressures of 71 and 68 in my eyes. A trip to the Eye Hospital and Emergency treatment reduce those and I was left on a regime of dorzolamide and latanoprost which seem to control the pressures.

In September last year I had bilateral iridotomies and I’ve been told that it’s not impossible for me to suffer from angle-closure my angles are so wide open. Clinic measured pressures have been in the low teens since. In February this year the dorzolamide was stopped. I had a follow-up two weeks later my pressures were in the high teens 18 and 19. A few days later I had the classic symptoms of a spike in IOP; cloudy vision and rainbows around bright lights. A trip back to the Eye Hospital where they could find nothing wrong. This was five hours later and my pressures had reduced by that point. there was a suspicion I had had a mini stroke and I was referred to a stroke clinic with subsequent heart scan, brain scan and all the fear and worry that went with that. Scans showed nothing and I was I have been discharged from the stroke clinic. Meanwhile I continued every few days to have the symptoms of high IOP that seem to be transient Nobody really knows what is happening. I was prescribed brimonidine in addition to latanoprost and dorzolamide and the incidence of blurry vision and halos reduced to about every two weeks. However my clinic measured pressures were still usually in the 20s. As a result I invested in the icare home tonometer at great expense and have been measuring my pressures for approximately one month. I have a follow up with the consultant on the 18th of July.

The data from the tonometer shows my pressures in the morning are typically in the low teens and in the evening range from high teens to low twenties to spikes of 30/35 and one day 45 At that time I had the classic symptoms of blurry vision and haloes. My question to this forum is, has anybody else had something similar? does anybody else have a huge range of pressures between morning and evening or day to day? thanks for any information, feedback or advice you can give

My sympathies. I went through something similar of intermittent halo vision progressing to cloudy vision. For me it might come on mid morning, last a couple of hours before subsiding. I felt it was often triggered by sunlight although I could be wrong about this. It became progressively worse, happening more often and for longer periods.

No-one was able to discover why this was happening, I have my own suspicions. Ultimately it was resolved with trabeculectomies and my eye pressures have been pretty stable since.

I hope you are able to get it resolved.

Thank you for sharing. Somehow it makes me feel better that I’m not the only one where this is happening. My consultant had no idea so at least I can take data to him. Will keep you posted

Wow , you have been through it , must be scary when they thought a mini stroke.

I hope you get some answers at your next appt 🤞🏻

It’s good that you have been able to monitor yourself , so can provide proof .

That’s quite a difference in iop ranges , so that does need investigating .

Hi, I have been doing home monitoring for just over a year now. I was really surprised to see how much IOP variation there is, within a single day and day-to-day. No doctors ever mentioned that to me, but this is common knowledge to them. My current doctor has told me that even with healthy eyes, an intra-day variation of 6 is somewhat normal, and for glaucoma eyes it can be higher. It made me realize that the single number you see at a visit to the office every 4-6 months is so limited. My daily highs were much higher that what I saw at the office.

Through home monitoring I have been able to work with my doctor to adjust medications to get the daily range much tighter. In addition, I have seen how daily stress and anxiety also can contribute to higher IOP, so have really put in the effort to address that.

Thanks. That’s helpful. This is my pressure chart since I started measuring about a month ago. Lowest is 9, highest is 45. I only measure twice a day, immediately after I get up and approx 12 hours later

That's a wider & higher range than what I had, but mine still looked somewhat similar. My range was probably 12-25 with occasional high numbers of 30. I added 2 more drops, 2x per day. Also adjusted the timing (morning + dinner time) to get the best effect. I've gotten it down to 11-16 most days.

I measure more than just 2x per day. Early on, I did a lot, to establish the normal daily pattern. Then was measuring to see what effects things like exercise, caffeine, etc. might have. And then along the way, I came to see that stress and anxiety has an impact too.

Hi there. I'm really interested in your posts. I have long suspected that my pressure fluctuates quite a bit - I know my blood pressure does so why would eye pressure be any different? Something else to bear in mind is that all the machines used to measure our eye pressure are calibrated differently so I think it is entirely possible that you would get different readings using different instruments. I know it's important to minimise stress but how difficult this is when you are undergoing a series of uncomfortable procedures to save your sight!

Interesting about reducing stress. I hadn’t heard about that before. As you say, it’s all very difficult when you think your sight is at risk, plus all the other challenges. Add in a stressful job and some critical issues with sick parents recently I haven’t exactly been living a stress free life. Just looked at your profile. Oh my, you have really been through it! Hope all is stable for you now
