So I am severely obese with high blood pressure that is controlled by atenolol.
I was just diagnosed via ultrasound while they were looking at my gallbladder. It wasn’t a Fibroscan but whatever they used gave them a reading of high moderate fatty liver.
My pcp isn’t concerned because all of my liver labs are mid-range normal e.g, AST ALT is 22/20 and my Fib 4 score that predicts cirrhosis is under the cutoff too. Liver contour was smooth, no lesions and proper blood flow. Apparently this all indicates no inflammation yet which is what causes scarring/fibrosis?
I was told it is hard to justify a Fibroscan right now with these results along with good cholesterol, no diabetes or pre diabetes and controlled bp of 118/80.
Im meeting with a nutritionist next week and the current plan is the obvious…lose weight, stick to the liver friendly diet and exercise. Pcp said we can redo the US in a year or get a Fibroscan then since the treatment will be the same right now anyway.
Any thoughts on this being a solid plan or should I be doing something else?

The treatment will be the same either way and since you don't have indications of advanced fibrosis it is a rational strategy. The question is do you have enough information to convince you stick with the lifestyle changes. One question, on the Fib-4 what cutoff was the doc thinking about? There is a high and a low. Below the low means you have a very low probability of having a fibrosis issue. In the intermediate, below the high cutoff, it suggests that you don't have advanced fibrosis but doesn't mean you are inflammation free. Doesn't change the approach but good to know what is really being said.
Hi there! Thanks so much for answering. I’ve been reading your replies to people all over this forum. You seem really really well-versed in this so I appreciate it. Newly diagnosed so as you can imagine, I’m scared to death right now.
As for the lifestyle changes sticking? I definitely think I can do it. I’ve been doing it for almost 2 weeks now and it’s so far so good. I’ve completely cut out anything with added sugar, stopped using any salt which is HUGE for me—I am/was a salt/fat pizza type of obese v sweets. I’ve added fresh, no starch veggies to every meal. I’ve also minimized the saturated fats and make sure I’m getting 3-4 tablespoons of EVOO every day to up the monos. Currently fitting nuts, chia seeds, blueberries and avocados in daily too. I love quinoa and chick pea pasta too. Main protein is salmon and cod but I know I have to find some variety with that. Does all this sound ok and putting me on the right track towards improving? Anything else I can add? I’m slowly increasing the exercise as I lose weight. I have a longgggg way to go. Dr is talking 18-24 months before I’m at ideal but I’ve lost about 35 since January 23rd.
I’m not entirely sure what end of the Fib 4 she was using. Do you happen to know the range?
It seems like you have it together. You took years to get here, expect a couple of years to get back to a proper place. The Fib-4 score below 1.3 very low probability above 2.67 a high probability so a fairly wide maybe zone. Here is some info that may help
