Hi there. I am not a massive fan of Lactulose. My 6 year old has had similar issues since he was 1. Lactulose never worked for him. Maybe you could ask your GP about Movicol. My Son is on a maintenance dose of Movicol he has 1 a day now. We have had lots of trial and error to find a dose that works for my son but all being well he is regular now.
I agree with Emmaxcx - my daughter suffered from constipation and stool withholiding for 18mths from aged 4 years old. She was prescribed Lactulose but got to the max dose quickly and it didn't seem to make any difference. We focused on diet changes. More fibre (Shreddies, Raspberries, Ryvitas etc) and more water. We did turn a corner but starting school put it back and we have now been prescribed Movicol and it has changed our lives. She has been everyday, sometimes multiple times a day for a month now which was unheard of before. We are slowly reducing it to find the right maintenance dose for her. Have you discussed a referral to a continence team? I would really push for this if possible, ours have been amazing and understand what is going on so much better than a GP. They might be able to help you break the cycle and advise much better on other things that can help.
Are you also doing things like drinking lots of water, trying for a poo about 20 minutes after main meals, blowing bubbles/windmills, using a proper toilet seat and high step to support his feet?? I’m sure you probably are but if not and you want to know more just shout we can point you to some good advice on this on this forum. I know it sounds like it won’t make a difference but it helped us so much more than I thought it would xxx
