Sorry to hear that you aren't feeling too well with the warmer weather. I think if you check the related posts to the side, there were some very good suggestions from other members on a similar post last year.
It might also be worth seeing your GP just to check your bloods if you haven't had them checked recently.

I know for myself that when it gets warm, I retain water more. It must be a natural reaction to the heat. When you can, put your feet up and I mean at every opportunity you can to elevated them. Try putting a bag frozen peas on the feet. You can refreeze them over and over ..... but I would not consume them if you use them for this. Make sure you mention this to you nephrologist.
I make my own ice pack...pour 3 cups water and 1 cup rubbing alcohol in a ziploc bag..× 2.....keep it in the freezer...because of the alcohol it makes slush instead of cubes and molds easily around ankles..toes...feet...etc....I reuse it over and over...it also soothes itching
