Is this proper to alter the daily dose of insulin on the basis of blood sugar test conducted at home. How frequently dosage can be changed

I am a diabetic for the last 17 years and on Tab (Galvus met) + Humalog Mix Insulin (12+14).

It is not a good practice. Frequent changes means the sugar control is not steady/

You have not given your glycemic levels. More over as you have been prescribed and using insulin of a particular mix, get the advice of your doctor who prescribed insulin of a particular mix for changing the pattern.
