Pseudo/Reactive? Really subjective hypoglycaemia!

Hi

I would like to hear from anyone who has this condition or similar, especially if they have found a way around it?

Background

Type 2 diabetic of about 20 years, currently 61yrs. High blood pressure, and sleep apnoea, both treated. When diagnosed I was about 130Kg, lost 30 of that around 10 years ago using fasting and lower carb, and I have kept it off. I am 180 cm, close to six foot. Now I am trying to lose some more weight to yet again lower my weight and hopefully to improve insulin sensitivity some more. I am taking Metformin and Jardinance. Since starting lo carb in April I have exactly halved my diabetic meds. Last HBA1C 6.9, down from 7.1.

The problem...

Every time I have tried to further lower my weight in the last 5 years I have hit the same problem, around 11:00 to midday I start to feel hypoglycaemic. And it can be quite bad, like some of the worst real hypo attacks that I have had, I am shaky, irritable, wobbly, off balance, anxious, stressed, but worst of all I simply cannot think or concentrate. Basically, anything I try to do to lower my weight will bring this on, extra exercise, low carb, fasting, I used to be able to fast with no problem at all for periods up to 48 hrs. Currently I am on low carb, just fasting from dinner to lunch causes this.

In an effort to understand this I use a CGM. I thought it was going to be that I was reacting to my morning BS rise falling off, but no! With the use of the CGM I have pretty much removed my morning blood sugar spike, so there is no fall - For me this removes reactive Hypoglycaemia as a cause. At the same time I have also prick tested myself and it agrees with the CGM, I am getting 2 agreeing BS readings from different parts of my body which should rule out Pseudo Hypoglycaemia. Before I flattened my trace this was occurring at readings between 7 and 8 or higher. With the effort I have put into lowering my BS it is now happening in the low 6's.

I have tried different mid morning snacks, high carb, low carb, the best strangely seems to be low carb. It seems the flatter my morning trace the less deep my hypo sensations mid morning, but this is leaving me feeling sub-par all the time like I have the (Keto?) flu. My limbs are weak, I am generally shaky, prone to tinnitus, off balance, and fuggy headed, generally like a low level hypo but without anxiety or deep cognitive block. Really like getting up and trying to walk immediately after or during a serious flu. (I should say I have been on this diet now since April, I would have expected to get over keto flu or any other starting factors)

Although my BP has lowered during my time on low carb, I do not believe this to be the cause, in fact the more hypo I feel the higher my BP, the anxiety I guess?

So in a nutshell, I get hypo around the same time every day with no measurable cause. And there can be times in the day where my BS can be lower and I feel relatively fine.

It seems right now that in an effort to improve my health I am doing exactly the opposite, every morning is a trial, I work writing database software for a large bank, try concentrating on that while hypo....
