It seems that Sinemet is not working. I get about 3 hrs on and the rest of the day off. I take a 50/200 @ bed and one during the nite. 2ea 25/100 when I get up and every 2 to 3 hrs during the day. What can I take to help extend on time.

The best advice I got from three different neurologists is take as much as you want or need for tremors until you get dyskinesia. I take about 2200 mg /day, but like everything else with PD it varies from person to person.

My neurologist kept increasing my dose of sinemet until I was taking 15 pills of 25/100 mg per day. This caused my left foot to jump with no control on my part. This scared me, so I started walking daily with a friend (2 to 4 miles per day). Within two weeks I was able to cut back on my sinemet to 5 pills per day and fewer PD symptoms. My new neurologist said sinemet works best if you take it right when you wake-up (have your meds on the night stand by your bed) and take as little as possible before bed and in the evening. This seems to help as well.
My next step will be to take the extended release version; that provides a lower dose, but extends it more evenly for a long period of time.

Are you on any other PD meds? I take Azilect and Comtan. These drugs enhance the length of time the L-dopa/C-dopa stays in your system. They make the Sinemet work better. There are perhaps a dozen other drugs that help too. You should ask your neurologist about them.
I take Sinemet, two in the morning, and two in evening. I don't know what it's supposed to do. I have no tremor, but balance is terrible. I walk like a drunk person, and I don't drink. My mind seems to be getting worse all the time. Can't think of names, forget how to get places - places I have known all my life.
