What on Earth is going on? Meds lasting 1 hour?

Hi gang. I hope that the title of the post says enough really, but to amplify a little: woke up this morning, took a rasagaline and one and a half madapar (150mg of levadopa). I planned some activities this morning but was amazed one hour later to find myself off again. This was taken on an empty stomach and no food was taken in that hour. Any thoughts most welcome.

It happens to me occasionally, sometimes with no rhyme or reason.

It can happen when the tablet(s) gets stuck in the folds of the esophagus or stomach. Drinking some sparkling water should be of help.

Levodopa works well but the delivery system isn’t great. Reliant on gastric emptying and competes with food for absorption. Even after that, the half life is about 1.5 hrs. Imagine if they could come up with one pill a day. (It might be a better use of research dollars, than dead end novel therapies?)

it could be that the body has to dispose of ldopa faster with a greater production of enzymes responsible for doing so, the so-called Amino Acids Decarboxylase enzyme (AADC also called DDC). This may be due to a change in vitamins and diet up to three days in advance. An example is grapefruit juice taken intermittently, see Wriga here:researchgate.net/publicatio...

I think I tried it when Albert first published but didn’t notice much difference. Remind me again of when to take in conjunction with the levadopa and how often if you will? Maybe I’ll give it another shot.

Try this take the medicine wait 30 min do not eat for 30 min.Then have small cookie 1-2.

Then for next 30 minutes eat anything you wish. Do not eat after

30. Repeat process for next dose.

No snacking in between.

Perhaps emptying the bowels well (or not, ie: constipation) has an impact? Just take note; it makes a difference for me.

Depends on how much is eaten night before sometimes. The digestive system is so sluggish. 😞

I've noticed it happens sometimes when I eat it big filet mignon the night before. And sometimes there's no rhyme or reason.

That has happened to me, Adam! For sure, a sluggish gut can contribute to absorption timing. One time, when I was going through a particularly difficult time in my life, grieving my Mom, and tensions were high, even though I took Sinemet regularly, stiffness and tremor symptoms were not well controlled! This was unsettling, because I had duties, as the eldest son, in my family, to attend to! It lasted for a few days. Fortunately, I have a very supportive brother, and sister, who helped me with setting up arrangements etc.

It happens to me when I have a gastro intestinal blockage. Or the other situation for me is when my body is fighting something else…. some virus , or bug out there. I might have no relief at all for a day or two!

after some light gymnastic i noticed that it startup faster and last longer. if i eat before 40 from taking it it may last less, because digestion takes energy
