I had a mandibular dislocation about three years ago. Doc and his nurse couldn't pull it back into place. A shot of Valium relaxed the muscles sufficiently that my mandible regained proper alignment. I slept well for ~3 nights.
As a dentist as well as a Parkinson's patient I have to commend you on an excellent video. You are correct that ill fitting crowns can cause TMJ problems. If they don't mesh correctly with the opposing teeth the smooth movement of the lower jaw can be impacted. Very often a simple bite adjustment can relieve the pain you describe. I have noticed that my jaw has been affected by my PD. I also have to be careful to not bite my teeth when eating. Thanks for your informative and we'll researched videos.

I’m currently also having difficulty with chewing on my tongue and inner cheek and lips. I also have a DBS unit. I think that the more I turn it up to control my foot dystonia the worse the chewing... it’s a consequence of the proximity of the motor pathways to the lower extremity and the glossopharengial nuclei in the brainstem. There’s a of micro current bleed over that produces dyskinesia of the chewing function.
I’m a retired physio- PT and have seen a fair number of TMJ headache cases. I think that it’s quite possible that it can be involved in both the symptoms and improvement process. But a word of caution... it’s a delicate structure so that it should be approached carefully. I also would consider the c-spine in postural stretching. The whole complex of shoulder, neck and TMJ are intimately related.

I find it spectacular and wrote to G. Demerjian if there is some evolution >>>> more PD-TMJ patients,, ,,,,more interest of neurologists,,,,, other dentists,,,, how are the results after three, fore years ,,,, until now no answer...
I went to Michael White (has worked together with Dr. Stack) in London, but unfortanely.....>>>> due to financial issue, I can't immediately go back.....that makes me so frustrated.
Because I have a big clicking on the right side and my symptoms are at the right side.
