I accidentally hit a key - I think it was the delete key resulting in complete loss of all answers. It took me ages to fill in this survey certainly more than 20 or 25 minutes. I am not going to bother going through all that again. Also please would it be possible to keep the headings visible while scrolling through the options / multiple guess etc

A very detailed and thought provoking survey. A good regular process to undergo, regularly. Pete is correct about scrolling, but nothing serious.
