does anyone here have the vh3-21 in their ivhg report??? How has this affected your disease progress????? Am borderline mutated-unmutated…. 1.96…and fish report was ok….. thank you so much for help. My dr talked more about the fish test being ok…. When it says poor survival associated with this vh3-21 just concerning… thank you 🌻

Hello Faith. My layperson understanding of things is that having mutated IGHV confers a favorable prognosis , but that being mutated at the vh 3-21 germline was more like having unmutated IGHV Cll, which carries a less favorable prognosis.

It’s important to understand, however, that unmutated Cll was a problem when chemo was the treatment of choice for Cll. With the novel drugs we have now, IGHV status has taken on less significance. Indeed is some cases, those with unmutated IGHV Cll outperformed those with mutated IGHV Cll. At a minimum, it can fairly be said that the novel drugs for Cll like ibrutinib and venetoclax have evened the playing field for those with unmutated IGHV Cll.

That’s all a long way of saying that the poor prognosis you read about for vh2-31 mutated Cll was likely based upon data from the chemo days of Cll treatment. The prognosis for most all kinds of Cll have improved significantly with the new medications available to us.
