how accurate are fib 4 scores?

Mine has some back as 0.96 which I think is a miracle considering how much I’ve struggled with alcohol over the years.

I’m just worried now that it isn’t accurate

Well I think it is a good start point but better to go to good doctor if you have other concerns about your liver.

From what I've read it's much more accurate at ruling out cirrhosis in people who definitely don't have it, but less accurate in ruling in cirrhosis for people who actually do have it. I've done all the different liver fibrosis related calculators which say I have either no Fibrosis at all, or at the worst mild Fibrosis (APRI, FIB4, GPR/I, NAFLD, RPR, RLR, NLW and AST/ALT) but I do have F2-F3 Fibrosis and portal hypertension so who knows. I wouldn't take them as gospel and definitely wouldn't use them as an excuse to think you can put off focusing on living better and looking after your liver... that's exactly what I did and it didn't work out great

Bananasincustard your reply is very interesting to me...I have a liver stiffness of 9.0 kPa and my GP is diagnosing me with cirrhosis...I feel certain I have F2....I did have ascites which led me to the Dr but was not ill at all...not tired, not sick...nothing and still feel healthy. Mine unfortunately is due to alcohol which I completely stopped immediately back in July. I had a nodular contour on my imaging at the start but the one I had Oct 30 and Nov 1 there was no nodular contour mentioned so I am praying with all my heart that means improvement. The last ultrasound said impression was hepatic steatosis but early cirrhotic/fibrotic change would not be ruled out due to a minimal contour irregularity at the subscapsular area...she is saying I do not have portal hypertension because my liver stiffness would be much higher if I did..I am doing like you say and just focusing on living better and taking care of my liver. All my bloodwork is in normal and better than normal range and came back in line very quickly. I did not know you could have portal hypertension with F2-F3 - I thought it meant you do have cirrhosis? Anyway I am going to just keep trying my best and pray that I am fortunate enough to be able to reverse the damage...only time will tell. I had severe fatty liver in the summer of 2021 but unfortunately the doctors did not tell me or I could have stopped this from progressing...my fault but had I known I could have resolved this so easily....and would have just like I stopped now :(
