Fibroscan and what affects them

while reading a lot of posts Ive noticed a lot of people having a fibroscan result saying they’ve got more damage than they really have. And have other tests to show other wise. What causes these overestimations. High BMI, above normal liver panels slight ones or really high ones,increased ferratin. And if some of these bloods are inflamtion markers and levels are known to Dr shouldn’t these tests be repeated after levels drop. I’m assuming this all affects liver stiffness. Thanks.

Conditions That Can Affect Your Fibrosis Results

Certain conditions can cause a liver stiffness result that’s too high, which makes it incorrect. You may have less scarring than your results suggest. This can happen if you have:

Liver inflammation (swelling): This can be caused by a recent liver illness. It can also be caused by long-term, heavy alcohol use.

Benign (not cancer) or cancerous (cancer) tumors in your liver.

Liver congestion: This means that your liver is too full of blood or other fluids. This is usually caused by heart failure.

FibroScan may give less accurate results, or no results at all if you have:

Obesity: This means your body mass index (BMI) is higher than 30 (a high, unhealthy amount of body fat).

Ascites: Fluid building up in your belly.

Biliary obstruction: A blockage that does not let enough bile flow out of your liver.

Scar tissue: Tissue from surgery or radiation built up near your liver.

Your healthcare provider may use imaging tests, such as ultrasound, CT, or MRI, to see your liver. They may use blood tests or a certain type of MRI to measure scarring and fatty changes in your liver. Call your healthcare provider if you have any questions.

is a enlarged liver considers swelling since many have enlarged livers with some of these disease.

You've kind of lost me there - liver inflammation/swelling/enlargement can be caused by numerous issues which can attack the liver. Having ongoing inflammation (often reflected in blood results) can cause false high fibroscan results.

Gotcha I didnt know if a persons size liver could elevate scores. I saw a video from fibroscan round table 2 Drs said if Alt and Ast and Ferritin levels were elevated that these could eleveate readings and test should be repeated when levels. Normalize .

"The threshold values also will be significantly impacted by the prevalence of the underlying condition. In Table 4, the effect of changing prevalence is demonstrated again allowing for appropriate choice of cutoff values depending on the clinical setting. These modeling data demonstrate that as the prevalence of liver fibrosis (≥F2 or F4) decreases, there is a commensurate reduction in PPV and increase in NPV

This is relevant as cutoffs generated in secondary care are often applied in primary care without taking into account the marked difference in prevalence. In this situation, a negative test would be very reassuring, although a positive test would have a low likelihood of capturing a true positive and raises the question of needing further confirmatory tests."

Yes I believe ultrasound. Blood test and fibrosis blood tests , fibroscan and any other tests required. Especially if they don’t all corelate.

I'm due to have a sheerwave scan ..my ultasound is now normal apart from gallstones.Bloods all great except ferratin which is raised 500 I think have been very anemic and I take iron supplements every day.

Are you suggesting this may affect my results. Confused 😕

Are you being prescribed the iron supplement? Too much iron is bad for the liver.

Yes I take ferplex 40mg everyday for the last 3 years. My hemoglobin was 7.6 ..I had blood transfusions 2 . Then 2 iron infusions .now daily iron. I've recently had a knee replacement bloods dropped to 7..6 again blood 2 bags then iron infusions now 10.6.

Im concerned about my ferratin they don't seem to be..originally afrer lockdown and drinking too much it was 2000 ..I feel very lucky fortunate to be ok. no cirrosis..but will this affect my scan I'm paying 250 euros as I need to know if I still have fibrosis. I'm having one on the spanish health system in Dec..I can't wait that long x
