I am in the same situation. My femoral is -3.2 but my spine is -0.3. I look forward to any explanation about bad distribution.

My understanding is that where there is major discordance, it should be queried. I requested a repeat when mine showed osteoporosis in the spine and normal hips but it wasn't forthcoming. It can be due to positioning errors but I don't know enough about it for further explanation.

Every segment of the vertebral spine bears a different amount of weight which is intensified during movement both proper and improper. Even with perfect body mechanics the vertebra sustaining the most weight will lose volume first if the elements to rebuild bone are absent. Our bodies rebuild bone constantly sometimes to repair tiny fissures but sometimes just to give us fresh bone. If something is amiss ( and inadequate diet or parathyroid issues...) the vertebra with the most weight will suffer first.

There can be many causes. Spinal disgenesis- developmental differences, spondylolisthesis--where one vertebra slips forward over the other, creating an uneven pressure; spinal stenosis, cutting the blood or nerve supply to the vertebra and causing awkward movements because of pain; disc herniation where the padding between the vertebra--the shock absorber allows more pressure on the lower disc; accidents from an earlier time in life that have left a vertebral segment more vulnerable.

I apologize because there must have been a short answer. Our bones are complicated. If you look at the picture below, the lacy bones are normal with all those porous spaces in between. Where the bone is broken reflects the loss of bone mineral density that gives us the minus on our dxas. There are two medications that can reconnect those gaps: Tymlos and Forteo. The other medications will make the other connections thicker and prevent the loss of more connections.
