Having recently been told that i have mitral regurgitation/thickened heart wall and awaiting an mri,, doctors have said I can live out my days (I’m 41) and there’s a possibility that I may not even need heart surgery. Only thing is I haven’t seen much evidence of this.
a large part of me suspects surgery is going to be needed 99.9% of the time.

I think it depends on what caused the damage. If it was rheumatic fever then things might be pretty stable. If it’s degenerative valve disease (what I have), then it’s only a question of how fast is it going to get worse.
I was diagnosed at almost the exact same age as you are; I had double valve surgery at 60. I believe my progression was considered unusually slow. 🤷
