HF meds and weight loss

hello.

I have been taking Entresto, dapaogliflozin, and Bisoprolol for just over 3 weeks and have had quite a bit of weight loss, half a stone and I am already very slight! I am eating more and not putting any on! Is this common with these medicines? I’m getting a little worried. Thanks

I suggest you ask a pharmacist or talk to the BHF Heart Helpline nurse who should be able to give a professional opinion on your circumstances.

I think you lose some weight with dapa through peeing your sugar out and entresto can have a diuretic effect so you pass more fluid. Obviously don't listen to me though, speak to a nurse.

Have the cardio department given you a non urgent number to call? I have one that I phone up and ask them about stuff like this or just phone up to moan at them about medications and stuff, you should have a local one too, find it out and give them a shout. Best to let them know just incase 👍

Thanks, I have heard that dapa can cause weight loss… have messaged my HF nurse too! Never thought I’d be complaining about loosing weight 😆

By inhibiting neprilysin, sacubitril increases the concentration of these peptides resulting in vasodilation (opening up of the blood vessels), natriuresis (an increase in the excretion of sodium in the urine), and diuresis (an increase in the production of urine)

Entresto isn't a beta-blocker or a diuretic. Entresto contains two drugs. One is sacubitril, which is a type of medication called a neprilysin inhibitor. The second is valsartan, which is a type of medication called an angiotensin II receptor blocker

Sacubitril has a diuretic effect then, its not taken as a diuretic... but could cause weight loss seems it has a diuretic effect, which is the point of this thread. Cheers

Hello, Since your HF diagnosis you've stated you've had no symptoms with HF but we do retain fluids so if you're taking diuretics you may have lost fluid on lungs that has caused your weight loss.

Whilst in hospital last September I lost 1 & a half stone during an eleven day period when I was given diuretics intravenously but I had severe oedema in my feet, ankles, lower legs, tummy & lungs.

Your HF nurses & Gp should be checking your lungs & the rest of your body for oedema to ensure there is no fluid build-up.

thank you. I had an xray in November and my lungs were clear. This was before I even started on the meds.

If you have no visible signs of oedema & your lungs are clear, then I would ask HF Nurses or your GP about your sudden weight loss & why you've been prescribed diuretics. Perhaps your sudden weight loss is due to the stress of you being diagnosed with HF?

I used to lose loads of weight whenever I was stressed, even when eating well.

I know many of us don't like to bother our HF nurses but you should put your mind at rest and give them a ring.

I have just started the dapaglifozin and been told it causes weight loss as you get rid of more sugar The drug was originally just for diabetes but works well for heart Have you had any sideffects I am slightly concerned about starting mine. I would watch yourweight if you are already slight a nd report it. I am sure others on it will be along with info which I will read with interest too

I started on Bisoporol and Spironolactone in September (lowest doses) and I too have lost half a stone. It does seem to have trailed off now and I remain within 1 or 2 lbs of this. Like you I am tiny anyway so even losing a pound or 2 makes a difference to my body.

I spoke to my GP pharmacist who wasn’t at all worried about it and gave me suggestions of calorie increase foods.

Thanks hopefully you will stop losing it I can afford to lose 7lbs so I don't mind that My body doesn't like them and always reacts and I end up having to come off them which is why I am worrying

Don’t be worried, apart from weight I have no other side effects and I feel good to know that this is helping my heart! Hopefully any side effects you may have will subside in time x

Hi, I have been on these 3 same drugs for 2 years now and I haven’t put on weight or lost weight but with having low bp it made me more dizzy. I can only take low doses of them for that reason. Perhaps you could question the strength of the doses you are on it might be too high for you - just a thought. Hope you get answers. Good luck.

This happened to me. I lost 2 stone in 9 days. It settled down after a few weeks though. It was just my body adjusting to the meds. Now I've gone the opposite way and put on loads of weight and can't seem to shift it! 😫

I lost quite a bit of weight from dapa despite my bmi being 25 to start with. I found eventually it to be unsustainable though. 2 years in I had to stop as I started to take an ill, gaunt appearance. Of my many cardiac woes HF isn't one of them so they allowed me to stop, the dapa was helpful rather than a must for me. I think it may be more important for a HF patient. What you are experiencing is totally normal in my experience but keep an eye on everything and discuss with the relevant medical professionals if you are worried.

Are you also on spironlactone that is water pill so will cause weight loss in water. Dapagloflozin although diabetes and HF drug caused water loss too. I am on them all. Try not to worry early days buddy.

Good morning....like yourself I am quite small framed and am on the same meds. I eat really well but am careful with regard to certain things so eat 'heart healthily'. I initially lost weight on the meds but have levelled out though have to say find it difficult to put weight on and would love to be carrying more. I believe it's the Dapagliflozin being that it is also a drug for diabetics so you don't retain sugars as you would normally do. If you have HF you are more likely on a diuretic as well. This isn't because you have any fluid build, it's to stop it from building up so that most probably also keeps weight down. If you are concerned, speak to your GP or HF nurse but I am sure it will be the dapagliflozin. Xx

It was too long but I'm still relatively slim because I eat a heart healthy diet which is low fats and sugars....you can't win can you lol. It just sometimes takes a while to level out on the meds. If you are really worried speak to your GP and see if the special dietary drinks they can give you would help. Might be worth a try. Xx

Dear Alim, I am a 72 year old Australian woman. I was already slightly built with low BMI ( weighing 46 kilos, height 158 cms) when I suffered an anterior STEMI 8 months ago. I had stents implanted, but initially had a low ejection fraction ( 36 ) . I didn’t have clinical signs of HF but was prescribed the standard meds for LV systolic dysfunction, including bisoporol, eplerernone ( similar to spironlactone) , valsartin, statins and blood thinners. I felt very unwell with dizziness, exhaustion and awful coughing and breathing problems and rapidly lost weight , getting down to 41 kilos despite trying to address it. I seemed to lose weight uncontrollably despite trying to increase calories .

After 2 months ,my ejection fraction unexpectedly returned to normal and many of the meds were reduced or stopped. I immediately felt better and managed to gain a kilo or two but am still pretty underweight. I do think the high volume of multiple medications ( especially eplerernone, valsartin and bisoporol ) were the principal cause of my weight loss, combined with the physical and mental impact of the heart attack. I hope that you too will be able to adjust and gradually regain some weight.

hi. I’m diabetic type 2 and have been on dapagliflozin now for a couple of years alongside metformin. The weight fell off me almost immediately because of the drugs mechanism so much so I tested for cancer but it’s a side effect of the drug for many. I’ve spoken with my GP on numerous occasions and in my case, anything that gets sugar out of my body quickly can’t be a bad thing. The weight loss has steadied but it was very noticeable for a while and I went from a 36 waist to 32 in no time.

I take the same. Started 2 or 3 y ago. Lost a good stone. Had to eat more. I believe Dapagaflozin is the reason. Causes you to pass sugars out of the body in your urine, I believe. Weight loss is an identified side effect

morning, I’ve heart HF as well and have been on my meds for a few years, the last one I was out on about a year ago was empagliflozin which is very much similar to dapaogliflozin and I lost about half a stone very quickly too but was told it’s a side effect of the drug as diabetes use it and it to with the sugars in your body. I have been on dapagaflozin, bisoprolol, ramipril and apixaban for almost year, no signs of excess fluid, lungs clear on last x-ray and if anything I've put weight on around the waist, get really bloated also and pee a lot, worse through night.

my husband took dapaogliflozin - he was already underweight and sadly within 6 months had to stop taking it - he was down to 7 st - it was either peg tube feeding or stop the drug . He stopped the drug, has taken him 2 yrs to put on 9 lbs

A great drug for lots of people but wasn’t for my husband

I would speak with medical team asap - there are nutritional build up drinks they can prescribe as well good luck

Hi. I am on the same medication plus a few others mentioned after a HA 2 years back. My weight had a quick drop and then slowed down and is now kind of stable.So over 2 years I went from 93kg to 81kg.

Generally I feel fine but I don't have any padding to keep out the cold!

I am on the same meds although recently I have changed bisoprolol to nebivolol as I blamed bisoprolol for giving me dizziness. I am 19 stone and on these meds now for 5 years. I have not experienced weight loss so in your case I dint think it's the meds. I hope you get to the bottom of the cause.Best wishes

No, not for me. I'm on the same plus more.Granted, I have 20 years on you, age wise.

My heart meds significantly lower my metabolism, as they should,but it makes losing weight almost impossible.

I know that's the reverse of your issue.

Can't give medical advice but suggest you speak with your Cardiologists.

lucky you!!! I've piled on the weight with Bisoprolol. Maybe its one of the other medications that making you lose weight.

Hi Alim in my case I lost around 9lbs I’ve been on these meds for around 4years now and have put on a couple of pounds over that time. I think it’s quite common but no harm in having a chat to your GP

Me too!! I'm on the same combo of drugs + a couple extra and I'm dropping so much weight. They found my HF when I was pregnant so once I started taking the meds it wasn't a surprise I was naturally going to lose some baby weight, although I really didn't put much on to start with during pregnancy. But it's been over a year now, and I'm smaller than I was before pregnancy.

I've always eaten really well and have a big appetite but I've always been quite small, so it does worry me actually how much weight I've lost/am losing! It doesn't feel like a good side effect to me!

Have you spoken to anyone or had any advice? Does it stop eventually and level out??

Hey, my HF nurse said it was probably the dapa.. it makes you pee out sugar and calories! I am the smallest I have ever been and not loving the look, I am eating a lot more to try and put some back on but nothing yet! It’s only been 4 weeks, loads of ppl have experienced this so I think it’s defo the meds and from others it apparently levels out in about 2 months x

Yes, it's the Dapa that does it. I'm on same meds and initially lost lots of weight but it did stabilize after a time.
