I had an ICD fitted in June of this year and it's nearly time to apply for my driving license, I have a form for my consultant to fill out but not sure if after they have filled it out do they send it back to me or send it straight to the DVLA as I have to fill out a form myself to reapply for my license.

Talk to the consulant's medical secretary to find out what they normally do, or better talk to the DVLA to see what they require.

I had a CRT-D fitted in Oct 19 and had the same issue. I spoke to my cardiologist who filled it in for me at my last check up before the six months were up and dated it for the six month time. He said he was happy doing this due to my recovery and general health. He the gave me the form back for me to send in.
I must point out that it was only about a week early when he completed the form.
If you surrendered your license rather than the DVLA taking it off you, you will be able to drive again as soon as they receive the completed form, before you get your license back. This information is on the DVLA site.

We asked my husband's EP to return the completed form to him so he could send it to the DVLA with his application form & his own completed form, we sent it all back by recorded delivery. He asked for it to be returned to him as he intended to drive under section 88 of the road traffic act & he needed to ensure the DVLA had all the paperwork first.
My consultant sent it straight back to DVLA. They’re used to it, as most of us want our licence back 😀
