Troponin

so I’ve had my troponin tested 3 times in 4 days. It’s gone from 3 to 4 to 5. Could that just be different factors my doctor said a lot of things can actually affect it. That I shouldn’t worry unless it came out at like 100s

Do you mean it’s come back in the minus as per your previous post?

healthunlocked.com/bhf/post...

If it’s minus 4 or 5, you’re more than within a normal range for your troponin. I assume you are talking about your high sensitivity Troponin T?

The doctor has said that because the higher the troponin, the more likely you are to sustain heart muscle damage and is indicative of a heart attack. Hence, why he probably said not to worry.

well you can’t have negative troponin. My hsTnT have been 3 then 4 then 5 but she said that’s fine

I’m not sure what you’ve been told, but in regards to your last post where you said your Troponin was <3mg/l, if your figures remain within that region, which seems like what you’re saying in your current post, that’s more or less considered as your Troponin being “undetectable” as I was told by one cardiologist. That is considered very good which is my understanding of it.

I think I just meant 3 idk why I said that the doctor said even though it’s gone up a couple bits she said it’s still well within range and it’d be a lot higher with a heart issue but you know my anxiety.

Sorry, I didn’t explain it well. I meant less than, as opposed to minus. As far as I’m aware, that is still well within normal range.

I completely understand the anxiety. When I had my heart attack, it was near the 100’s. Your results show that you are not even close to that number.

How are things going with your anxiety? Has your GP referred you to speak to a therapist?

yeah I’m finally in therapy starting Monday. And starting an antidepressant very nervous

That’s brilliant news! It’s always a little bit nerve wracking when taking a medicine we’ve not had before. Fingers crossed it will help with some of your anxiety symptoms.

Let us know how you get on with it.

In the UK troponin blood levels are measured in ng/L.

A troponin blood level below 5ng/L suggests that the person is unlikely to have had a heart attack.

For more details see Prof Nick Mills research into high sensitivity troponins.

researchgate.net/scientific...

His research has shown That a 'myocardial infarction could be safely ruled-out in all patients presenting with chest pain ≤3 h when using a single hs-cTnI value <3 ng/L as diagnostic cut-off.'

Prof Mills is a BHF funded researcher and is doing further research, using artificial intelligence to improve the diagnosis of patients experiencing a heart attack.

My doctor said tachycardia can even cause troponin spikes but you know my brain

Milkfairy profile image
MilkfairyHeart Star in reply toDshadzz07

That's correct.

Some of the causes of raised troponin in the blood:

Rapid heart rate.

Infection in particular sepsis.

Inflammation eg. Myocarditis

Blood clot such as a pulmonary embolism.

Cerebral haemorrhage, stroke,

Kidney disease,

Extreme exercise, marathon runners can have raised troponin blood levels at the end of their run.

Trauma and after heart surgery .

One lovely Cardiologist said to me you can't be a little bit pregnant but you can a little bit of a heart attack 😊

Yh my heart rate has been like 120 from the hyperthyroidism and intense palpitations. Just trying to think logically that I’m 21 and the dr said it’s nowhere near the cut off for a heart attack

Have you seen an Endocrinologist about your hyperthyroidism?

Your hyperthyroidism could be contributing to your feelings of anxiety.

it was hypo but now is hyper. It’s kinda weird right now.

It's way too hot. Lot's of typos today from everyone 😂

I suggest you ask your GP to refer you to an Endocrinologist to see why your thyroxine levels are so variable.

Looks like I’ve made a typo, I meant “ng/l” and not “mg/l”.

Yes, I remember you attaching an article to the OPs post the other day about how Troponin is now measured. I don’t think this guideline is widely practiced/rolled out in all relevant healthcare settings just yet?

Dshadzz07 profile image
Dshadzz07 in reply toTos92

I just got told anything above 14ng/l is when they retest. You know just how it’s gone 3-4-5 I’m like scared about that but maybe it’s just from my stress. Like can’t be much difference between 3 to 5

Ah, the problem of the translation of research findings into practice 🙃

So don't worry then!

When I had my last takotsubo event my troponin level went up to 79 and I was told it was a mild leak.

Dshadzz07 profile image
Dshadzz07 in reply toSilvertail

so I shouldn’t worry about 5?

Silvertail profile image
Silvertail

I wouldn't think so, but don't take my word for it. You may measure it differently in the UK.

I can put your fears into perspective, my troponin level baseline is around 140 and has been for around 4 years, it was only explained recently when my third cardiac MRI identified scarring on my heart muscle, I am now classified as having heart failure after 7 angiograms in as many years. When I had my first MI in 2015 my troponin levels were 980.

Dshadzz07 profile image
Dshadzz07 in reply toRosey666

obviously I’m so sorry about what you’ve been through but I also thank you for putting into perspective that 5 clearly isn’t that high in the grand scheme of things. I really hope things get better for you.
