Inclisiran

Following diagnosis of very high LPa I have at last been approved for the injectable Inclisiran cholesterol lowering drug due to my low intolerance of statins and bempedoic acid. This drug has been approved by NICE and the NHS and was booked in for my first injection in November 22. Unfortunately I got a call from my surgery the week before to say that couldn’t administer it now as they had received notification from the LMC that they weren’t allowed to. I spoke to my Doctor who stated it was down to resources and he was just the messenger, I also spoke to the LMC who were very helpful and explained that Inclisiran is not on the list of medications that are under surgeries contract to administer, which she explained was basically they weren’t getting paid to administer the drug. This is very frustrating as this all down to money, who is getting paid to do it whilst the patient suffers. Has anybody else experienced this?

If its any help earlier this year I asked my GP practice if they were in a position to administer Inclisiran with a view to to possibly having it prescribed. I was advised that they had received no instructions from the NHS about Inclisiran, that staff would then have to be trained in its treatment programme, and I would then be assessed.

In view of that response I then contacted the Local NHS Integrated Care Board who oversee GP Practices in my area and was advised that GPs had received the green light* to administer, and I should again contact my GP. However I didnt see this through since my annual cholesterol test showed a significant improvement and took me out of the Inclisiran 'zone'.

So all I can suggest is you find out who manages and oversees your local GP practice at the next NHS tier up and see what they have to say.

* NHS 'Green light' in my area is

Green

Initiation : Any care setting

Repeat Prescribing : Any care setting

Monitoring : Any care setting

Comment : Suitable for non-specialist initiation

Just a thought, could you ask to be referred to a lipid clinic at a major hospital for it to be administered there? No doubt there would be some duplication of all the tests you have already had but it could be a way round the GP funding issue.

Best wishes

I have been approached to attend a clinical trial named Orion4, I have a hospital appointment next Wednesday to see if I am suitable. I have spoken to my GP and he was very positive about it. I don’t know if it’s the same drug you’re talking about but he stated that he’d administered an injection for Cholesterol to 3 of his patients and had been told that only Consultants were allowed at present, will wait and see.
