Its a clinical coaching service for patients with long term health conditions. To give patients support,and to try and prevent non elective hospital admisions. And to free up doctors valuable time.🤔 You can find out more on there website,bit more to it than my brief description. Looks like a great idea, for anyone it may apply to,and benefit. 🙂
