Brachy- Tachy Syndrome ?

Hi all,

Just wondering if anyone has heard of this or has this? Another name is Sick Sinus Syndrome. I have reached my own diagnosis at present. I shall run it past my cardiologist very soon and see what he thinks considering he is quite baffled by me and has asked for advice from colleagues.πŸ€”

It is my left ventricle that is not working right/nor my electrics and my HR can bounce from 146- 70 or 135 to 80 then 69 like yesterday on my fitbit watch. The lowest it has been is 35 the highest is 157. I know it isn't AF or SVT as it doesn't stay bouncing on one figure..

My husband is being investigated (of sorts) for sick sinus syndrome (SSS) at the moment, we're off to Papworth for more answers on weds, if it doesn't get cancelled.

I wouldn't trust your fitbit, it cannot pick up missed beats, neither can a BP monitor so you need to do the HR manually.

My husbands resting HR can range from 48 to 70's so well within range. The reason why it's thought he may have it is because his HR will beat normal, slow right down & then pauses, a real solid gap before the next beat. It then starts again either beating normally or a fast beat. Other times, his beat just pauses, other times, he has what I think is known as ectopic beats & at other times, its just a normal hr albeit in the 40's - 70's. We monitored his hr for 2 weeks before contacting his GP. Does this sound anything like you're experiencing? I think SSS is really the heart rhythm doing a number of different things that are not normal, GP said it sounded bradycardia & svt's hence why he thought SSS.

Thank you for your reply.

Yes, this sounds like me. I guess I bought a fit bit just to see. But I have figured out it can miss beats as it moves up and down my arm..

Yes, my electrics are faulty so i already get missed beats infact it feels like it stops and sometimes goes into a flutter. I have been known to do a tarzan to jump start it back into sync. I do look odd banging my chest a couple of times. Then it pumps really hard and just dips. My resting HR manually is between 69-82 then everything else in between.

It makes me dizzy, lightheaded some days feels like I zone out even though I don't. My BP is no more than 103/63 most days all day as I have checked periodically at different times.

I am waiting on my holter results which will hopefully show this. They are sat with one secretary at a different hospital to where my cardiologist is. He is also only doing telephone consultations which is hopeless when you are deaf. So I am hoping to ask him if he will do an email consultation as I have written to him a few times, (pacemaker has been mentioned as a possibility on his replies) after he has my full history.

His secretary doesn't know when I can see him face to face which is not much cop either with these masks but needs must..

Good luck I hope it isn't cancelled.

The banging could be palpitations? Could your low BP be the cause of your dizzyness! My husband BP is reading between 94/68 & low 100's/over 60's, fortunately he's not had any dizzyness but he's not aware of the different beats either, I picked them up doing his pulse.

I think pacing is the usual method of dealing with SSS, although they were talking of doing this previously because if his low HR. Are you on a beta blocker?

I hope you get the results of your holter soon and can sort out your consultantion, I'm sure they're still seeing patients that they're unable to have phone/video consultations with. I'm not very patient unfortunately!

My husband would normally see his cardiologist this month but not sure what's happening, I suppose we'll know more after we've been to Papworth.

No,it's not palpitations it is erratic beats that skip/jump etc.. I take no tablets as I have multiple drug intolerance syndrome. Falling to bitsπŸ˜‚.

Good luck and I hope hubby gets the right diagnosis.😊

I thought I'd just let you know that the appt with Papworth is definitely on, apparently its noted on the paperwork that's it's not to be cancelled!

Definitely chase those results, good luck.

I was dx initially by one or two cardiologists after my first a fib attack with sss, it turned out it was a side effect of the 600 mg proprafenon that i was given in ERbut it lasted nearly two wks

then

I developed fist degree heart block again it was a side effect of bisoprolo that used to cause the missed beat and the prolongation of p-r interval

my point is try to get more than one opinions and don’t rush into any intervention before you make sure that your cardiologist and your EP are very good

Good luck

Duly noted.

My cardiologist is referring me to one of his colleagues if he doesn't know what is best for me. I might need an electrophysiologists basically a (cardiologist who has done 4 more years and specialises in electrics).

I have af but also tacky and brady, I take beta blocker to control the fast beats and have a pacemaker to control the slow beats.

I can't tolerate tablets I have multiple drug intolerance syndrome I had to come off the standard package too many serious side effects. So it's deep breathing at the minute when it guess too high and I start to feel weird.🀨

Hi, all I know is Brachy means slow heart rate and tachy means fast heart rate. I have had both and neither are nice. My ablation sorted all that out . Go luck stay sake and well

Good luck πŸ‘ maybe one day we will get to meet up in the waiting room, or just as good get a diagnosis and be sorted sometime soon.

Very similar to my heart rate symptoms, left ventricular systolic dysfunction, somewhat resolved with pacemaker implant 18 months ago. First serious symptoms were syncope episodes which were often associated with low blood pressure. This resulted in stopping bisoprolol until pm was fitted. If you have any fainting symptoms contact the cardiologist by phone (as I did) for advice and alterations to meds. Good luck

Thank you for your advice. I can't take tablets. My cardiologist knows this. I am also deaf which he also knows. I am waiting on an appointment face to face which could take some time.(Covid 19). It has taken 1 year already to get this far and I have gone from mild/moderate. Time to push to the top of his list I think..

Yes I have this. Mine is the result of a congenital heart defect in my electrics due to a faulty gene. It's quite rare. Naturally my heart rate was low and then it would randomly shoot up but more importantly my electrical impulses were coming from the wrong places in my heart. Was very scary for a while there. I'm being treated now but it's a bit of an unknown. Good luck with your diagnosis 😊

Brandibell profile image
Brandibellβ€’ in reply toSpatz76

Thank you for your reply. That sounds just like mine and they suspected i might have a unknown congenital birth defect on my first admission via echo cardiogram. Can I ask how you are treated? As I can't take tablets.

Spatz76 profile image
Spatz76β€’ in reply toBrandibell

It's a little like a needle in a haystack trying to find the faulty gene but the cardiology teams know a lot about the main ones that cause problems so if it is something like that they will no doubt find the problem from a simple blood test. I am being treated with Beta blockers to reduce my heart rate to help with the fast rhythms and then an ICD which paces my heart to ensure it doesn't go too low. As its naturally already low (was in the 40's) and also to protect me from the ventricular tachycardia I was having from the bottom chambers of my heart. These were the rhythms I'm being protected from. I was scared of getting an ICD for so long but now I'm a couple of years down the line I've forgotten it's even there. Something I never imagined I'd say. It also makes me feel much safer.

Brandibell profile image
Brandibellβ€’ in reply toSpatz76

Thank you for replying I think as much as I am hoping I might not need an ICD I think that is going to be my solution as my LV is not pumping enough blood around and my HR can drop to 35. I also cannot tolerate tablets so who knows for the Tachy beats what the solution is.

It scares me though as I am left handed and I am a single parent and I have heard they need fine tweaking, i am not very big so not much extra fleah around my collar bone and especially in the middle of a pandemic..haha. I shall have to wait and see and remain positive up until then..πŸ™‚Take care

Interesting everyone seems to take either tablets or an ablation? Which I am clueless about for a fast beat and an ICD for a slow beat.

My feeling was they just set an ICD at say 60BPM and away you go.

Obviously still Naive and New to all this Heart Business a year later.

I just wish it would go away then I can get back to having a life instead of sitting in Limbo..πŸ€·πŸ½β€β™€οΈ

Next.. .πŸ˜‚πŸ˜‚it has gone in long enough.. .#PMA

I had a few halter monitors on over a few periods of time to see if they could pin point where my tachycardia was coming from but it seems it never happens at the same place twice. Or certainly in no particular pattern. So unfortunately ablations weren't an option for me. Worst luck! πŸ˜ƒ

I have recently just had one a week ago.( First one). I am waiting on the results and I have a feeling mine might just do that. My HR goes from 35 lowest to RH 69 ish right up to 149 in any given day. How long should I wait for My results would you suggest give or take with Covid at the minute?.

I have low heart beat (30) and hight heart beat 150-200. Have a peacemaker for the low and medical for the high heart beat.

Seems to be a common pattern with the ICD it is basically to keep the HR up to a certain beat. Thank you for your reply.. e

Morning, im just reading some previous post to get some information that might help me. I've been diagnosed with mild lvh, bradycardia and mitral valve prolapse with a low resting hr of 33 which can go to over 175+ for no reasons. After 2 years and many test I have been taken off bisoprolol so the cardiologist can see my true hr. It's been 6 wks now and my pulse still drops to 38 or shoots up to over 175 and I'm getting more palpatations and irregular beats. The cardiologist has said if my pulse stays low he will fit a pacemaker so having another holter monitor for 3 days next month to see what happens next as he doesn't want me back on bisoprolol with me having bradycardia but it was helping me out with the lvh.Any advice or own personal experiences on how to live with a pacemaker or icd?.

Regards

Taz71.

Low hr with no meds.

I will not be making or agreeing with anything until all avenues have been explored including an electrophysiologists. As a cardiologist just tends to deal with the plumbing/stents/HA etc..

I totally understand, it is a big decision. Once it is in thats it..Mine is more cautious I feel. He wants a thorough History from the hospital I was first treated in and from me and then if needed he will refer me to one of his colleagues to discuss a pacemaker..
